DISCHAGE PLANNING
 Advise the client to take the prescribed
medicines regularly on exact dosage, time and
MEDICATION frequency and educate to not skip or stop taking
abruptly.
 Provide a simple explanation about the purpose
of the drug regimen.
 Encourage the client to do a moderate exercise
for at least 15- 30 minutes every day (walking or
Zumba) but advise to avoid strenuous exercises.
EXERCISE  Advise the client to always keep herself hydrated
(before and after exercise) and to bring extra
fluids.
 Encourage the client to always maintain a
healthy weight appropriate for her age.
 Emphasize the importance of adhering to the
drug regimen.
 Emphasize the importance of proper diet,
TREATMENT exercise and adequate rest to treatment process.
 Instruct the client to comply appropriately to
every health teachings of the physician and
healthcare provider.
 Encourage the client to always monitor her sugar
(glucose) level and blood pressure.
 Advice the client to have an adequate rest as well
as sleep approximately 8- 10 hours per day.
 Advice the client to always maintain a clean and
HEALTH EDUCATION safe environment.
 If there’s a presence of wound/ sores, educate the
client to keep it always clean and dry and
monitor for presence of infection.
 Educate the client about the signs and symptoms
that needs immediate actions and referral.
 Emphasize the need for a regular and follow- up
OUTP- PATIENT/ REFERRAL checkups.
 Advice the client to call the physician or the
health care provider when signs and symptoms
are again being experienced as well as the danger
signs that needs immediate interventions.
 Advice the client to follow strictly a diabetic diet
(low fat and low calories).
 Encourage the client to eat a balanced diet
(minimum of 3 meals per day).
 Advice the client to increase the intake of fresh
DIET fruits and vegetables as well as fiber rich- foods.
 Advice the client to maintain an adequate fluid
intake (2500mL/d).
 Encourage the client to avoid foods that are high
in salt such as processed foods.
 Encourage the client to avoid caffeine and
alcoholic beverages.
 Encourage the client to continue his religious
SPIRITUAL activities (if he have).
 Encourage the client to seek spiritual support and
always have faith in God.
