Thank you for your recent order. Please be advised that Oklahoma Vital Records requires the following
identity verification documents to process your request.

Instructions For Gathering Required Documentation

Gather a form of identification from list A or B below. All identification may be


subject to verification with the issuing authority.
List B - two documents are required:
List A - only one document is required:

Concealed Carry permit Marriage certificate (Certified English Translation, if applicable)
Resident Alien Card (Form I-551) Car registration, title or insurance/security verification form issued

2. Be sure the following information is visible on digital images of
identification: Full name and address, signature, license or
identification number, and expiration date
