Incident Investigation Report

1. An incident occurred during excavation work for a storm water line in Zone 1, Area 2 of the project site. 2. The excavator operator accidentally hit and damaged an existing fire hydrant pipe line while excavating near it. 3. The site engineer and managers were immediately notified and the damaged pipe was repaired by the municipal authorities. No injuries were reported.
