The affidavit states that Ahmed Omar Bawazir, a Tanzanian living in Morogoro, has used that name his entire life and is known by it. However, due to an error when registering his SIM card, the name Ahmed Shabibi also came into use, which has caused inconveniences. Bawazir affirms that he has always been known as Ahmed Omar Bawazir.

AFFIDAVIT AS REGARDING TO THE NAMES OF AHMED OMAR


BAWAZIR

I, AHMED OMAR BAWAZIR of P.O BOX 685 Morogoro Tanzania do


hereby affirm and states as follows;

1. That I am a Tanzanian currently living in Morogoro.


2. That during my life time I have been using the name of AHMED
OMAR BAWAZIR.
3. That my whole life I have been known as AHMED OMAR
BAWAZIR
4. That the name of AHMED SHABIBI came into being following the
event of errors during the registration of my SIM Card.
5. That by using those two names it caused inconveniencies to those
who knows me personally and in my usual business conducts
6. THAT, I make this declaration conscientiously believing the same
to be true and in accordance with the provisions of the Oaths and
Statutory Declarations Act Cap 34.
