ABOVE CEILING WORK PERMIT

Display permit in a prominent location at the work area at all times once the work has started

Instruction:
1. Permit can be obtained from Monday to Friday from 7:30 am to 2:00 pm.
2. Complete the fillable above ceiling permit work request.
3. Allow the safety at least 24 hours to complete the approval that includes actual
inspection and assessment of the area.
4. Above ceiling work permit are returned after the activity.
