AFFIDAVIT OF ATTACHMENT OF SIDECAR

I, SERVANDO BAGGAY SALAZAR, of legal age, Filipino, married and a resident


of C-12, Zone 03, Namabbalan Sur, Tuguegarao City City, after having been duly sworn to in
accordance with law, hereby voluntary depose and say, that –

1) I am the registered owner of a motorcycle which is more particularly described as


follows:

Make: Honda (TMX125) Year Mode: 2020


Engine No: KSW10E386188 CR No. 39579087-2
Chassis No. KSW10384317 MV File No. 0201-00001048175

2) Recently, I have attached a sidecar to said motorcycle unit, which attachment was not
included in the registration;

3) I am executing this affidavit to change the Body Type of said motorcycle to


Motorcycle with Sidecar and attest to the truth of the foregoing and in support of my
request to change the entry in the Body Type section to Motorcycle with Sidecar in
the motorcycle’s OR/CR.
