How To Use Your Provider Directory
This is a list of health care providers you may go to. A Primary Care
Provider (PCP) is your main doctor who takes care of you. Call the
Humana - CareSource™ Member Services Department at 1-855-852-7005
(TTY users: 1-800-648-6056 or 711) if you need help reading or
understanding this list. It can be read to you in English or in your first
language. Written translations may be possible. It may also be printed in
some other way if you have trouble seeing.

The data in this list is up-to-date as of 12/5/2013. The data can change. We
check and update it often. You should always check with the provider to
make sure. Call the Humana – CareSource Member Services Department
at 1-855-852-7005 (TTY users: 1-800-648-6056 or 711) if you have any
questions.

This list is split into sections by county. You can go to any provider no
matter what county they are in.

If you don’t find the provider you’re looking for listed in your county, please
check neighboring counties in this list. Or look on our website at
Caresource.com/KY. Select “Find a Doctor/Provider” under the Kentucky
Member tab. Or, call Humana - CareSource’s Member Services
Department at 1-855-852-7005 (TTY users: 1-800-648-6056 or 711).

Primary Care Providers (PCPs)

Each Humana – CareSource Member must choose a PCP. Your PCP is a
doctor, or a doctor group. Your PCP could also be an Advanced Practice
Registered Nurse (APRN) or APRN group. Or your PCP could be a
Physician Assistant (PA). Your PCP is trained in:

 Family Health Care (general practice); or

 Internal Health Care; or
 Child Health Care (pediatrics)
