Customer Request Form

1) The customer has submitted a Customer Request Form to SVC Co-Operative Bank for modifications to their account details including a change of address, updating contact details like mobile number and email, updating date of birth and identification documents like PAN and Aadhar. 2) The customer has also requested account related modifications like a change in account operation mode, adding or deleting joint account holders, updating power of attorney and nomination for the account. 3) The bank acknowledges receiving the form and notes that the requests will be processed within 8 days along with supporting documents. Certain online services will be blocked or deregistered due to the changes requested.
