Please check the appropriate box below to describe verbalized or demonstrated competence level using the following
criteria:
0 = Did not Perform 1 = Not Yet Competent 2 = Competent

PROCEDURE 0 1 2 Comments
1. Check for the scene safety (move to safety if necessary)
2. Wear clean gloves (if available).
3. Check for person’s responsiveness by tapping both shoulder while shouting
“Are you okay’?
4. Quickly scan for any signs of breathing (visible rise and fall of the chest.)
5. Activate Emergency Response System (Call 911).
6. Place the heel of one hand on the center of the chest.
7. Place the heel of the other hand on top of the first hand, lacing your fingers
together.
8. Keep your arms straight, position your shoulders directly over your hands.
9. Push hard and fast:
A. Maintain a rate of 100-120 compressions per minute
B. Maintain a Depth of at least 2 inches (5 cm)

C. Allows the chest to completely recoil after each and every compressions.
10. Continue Compressions until one of the situations are present:
A. Saw an obvious sign of life (E.g. Breathing)
B. Arrival of Emergency Medical Responder
C. Automated External Defibrillator is ready to use
D. If you are too exhausted to continue
E. Scene becomes unsafe
