Description of activity : Confined Space Entry into the tank to carry out Internal Activities Dept. / Div.: Operation

For confined space with multiple manholes (entry points), entry shall be restricted
by providing cautions board or warning tape (cross barricade) or controlled by
having additional attendant(s) with proper communication among the attendant.
5 Asphyxiation Ensure Oxygen test range from 19.5 to 23.5% to work without BA. 2 3 L Issuer

Mechanical ventilation is prefered over natural ventilation.
First entry shall be done by only one person using Airline mask, entrant shall
perform gas test at different locations inside the tank
Keep SCBA as standby to use in case of emergency
Executer shall ensure that workers take interval breaks to go out of the tank to
prevent fatigue
6 Poor illumination Adequate illumination shall be provided using 24 volt flameproof lamps. Also 2 3 L Issuer
lighting used shall be positioned to enable the attendant to see clearly the entrants Executor
working inside the tank.
Light intensity shall be measured using Lux meter to ensure adequate illumination
more than (300 Lux).
7 Electrocution Proper function of ELCB (30mA) shall be ensured before start the job. 2 3 L Issuer
Don’t use cable with multiple and loose joints. Executor
8 Slips, Trips & In case of work at high elevation inside any confined space, appropriate fall 2 3 L Issuer
Falls protection [Safety harness with double lanyard / Proper edge protection (guard Executor

The materials / electrical cables / Hoses shall be laid properly.
Adequate illumination more than (300 Lux) shall be ensured.
9 Heat Stress Drink sufficient cold water. 2 3 L Issuer
Modify work schedules and arrange frequent rest periods with water breaks in Executor
shaded areas.
If the worker is not alert or seems confused, this may be a heat stroke. CALL 888
IMMEDIATELY, Move the worker to a cooler/shaded area and apply ice as soon
as possible.
10 Night Work Avoid working alone and always maintain the proper communications with
hazards coworkers. 2 3 L Executor
Sufficient illumination shall be ensured in the work area.
Safe path to/from work location shall be ensured.
Barrication and Flashing lights shall be ensured in the excavated area.
Provision of retro-reflective stripes on FR Coverall must be ensured.

Provision of retro-reflective tape on equipment shall be ensured.
Don’t work in a dark location to avoid reptile and insect bites.

Explain emergency evacuation procedures to ensure workers can escape easily in
case of emergency. (Follow the wind sock cross/up wind direction to reach the
nearest transfer point).
Ensure that workforce has understood all the precautions properly.

Notes:
1. CTRA contains only those hazards and controls which are not captured in SWP Checklist and which could not be captured in Work Permits
2. Only competent (knowledgeable, skilled, trained & experienced) personnel {personnel who are competent to do this critical work} shall carry out this critical work; This
CTRA becomes invalid if this requirement could not be met
3. Implementation of all controls specified in Work Permits, SWP Checklist and CTRA is mandatory to ensure safe completion of this critical work
4. CTRA reference number shall appear in all pages of CTRA
5. Master Copy (original) of CTRA shall be retained in file for records and future reference
6. Photo copy of CTRA shall be attached with work permit
