Ⅳ Daily Construction Cycle

1. Inspection and observation prior to start work
➢ Work requiring PTW should be inspected and approved by the construction manager

➢ Designated high risk wok and the risk assessment ,must be inspected by supervisor , subcontractor manager
,construction engineer ,construction manager and take corrective as required also ensure record the finding.
➢ PM and HSE manager /Officer should conduct site patrol at all worksite and manage record keeping .
3. After work
➢ Subcontractor manager should report the result of inspection and observation to the construction manager after
work

Permit to Work Hold Point Risk Assessment (RA)


1. For any high risk/non-routine work activities, - Hold point 16 nos. work items(Total 34)
PTW system must be followed. Working at height, Hot Work, Unauthorized
2. A permit to work system can be valid for only Entry, Crane Movement, Lifting, Opening over
twelve hours. Structure, Scaffolding etc.

• The project constitutes an emergency rescue leading group with a team leader, three deputy
team leaders, two safety officers, one firefighter, and several rescue teams.
• The team will coordinate its emergency rescue work with the leadership of the emergency
command headquarters of the main contractor.

Emergency Response: Self-Rescue


If the person working at height has properly selected and used his/her fall protection equipment, the self-rescue includes these steps:
•The rescue ladder will be lowered to the fallen worker, who will grab and climb back up to a safe level.
•The worker will be evaluate for possible medical attention.
•All components of fall arrest system impacted by the fall event will be collected for inspection, service and documentations (bag and tag). Name, date
and activity at time of fall shall be recorded and the equipment handed over to management.
Roles and responsibilities of the hazard assessment team:

i. Conduct risk assessment according to the characteristics of the construction site and the
hazards of the production safety process;
ii. Provide guidance to the safety team to implement and monitor safety measures to reduce
and avoid accidents from dangerous sources.
iii. Ensure accurate risk assessment data and information of hazard sources to provide a
reasonable and accurate basis for emergency response assessment;
iv. Implement effective social communication resources for emergency response and provide
timely emergency response support measures;
v. Determine the location of the emergency response on-site command center where various
accidents may occur so that the emergency response can be activated promptly.

vi. Formulate emergency response materials, equipment and manpower plans.


❖ Roles and responsibilities of on-site medical rescue team:
i. Rescue injured personnel at the scene;
ii. Transport the injured personnel to the hospital where appropriate;
iii. Assess the personnel’s injuries and determine the most appropriate first aid.

❖ Roles and responsibilities of emergency rescue team:


i. Organize personnel to facilitate rescue work on site;
ii. Set up on-site fire brigade;
iii. Ensure smooth channel for rescues teams on site.
iv. Control the development of accidents and eliminate dangerous situations.

❖ Roles and responsibilities of the Post-Emergency Working Group:


i. Establish effective communication to provide updates to Casualties and their family members.
ii. Ensure proper tracking on medical rescue of injured personnel, coordinate and deal with any conflicts
that may arise with the relevant medical rescue department;

Assist the insurance department on compensation for casualties and property losses;
Roles and responsibilities of the accident investigation team:
i. Preserve & protect the accident scene;
ii. Taking samples and sealing of relevant physical evidence on site;
iii. Investigate and understand the main cause of the accident and the responsibilities of the relevant
personnel;
iv. Provide an investigation report on the accident & recommend remediation actions where appropriate.

v. Provide assistance to local authorities where necessary.

❖ Rescue materials and equipments:


i. Assess the nature and characteristics of each construction site and determine the actual needs of emergency
rescue work are equipped with emergency appropriate rescue equipments;
ii. Emergency rescue equipments are regularly maintained and are always ready for use in emergency
situations. Example of emergency rescue equipments: first aid kit (equipped with commonly used drugs), fire
fighting equipment (fire extinguisher, fire bucket, fire sand, etc.), lighting equipment (hand lamp, flashlight,
etc.), transportation(vehicle), other rescue equipment (stretcher, etc.).
According to the characteristics of large ceiling construction, there is higher probability accidents
occurring. For example, falling, object strike, mechanical injury, electric shock, collapsed large
mechanical equipment, etc.

1. Commencement of the plan: Emergency plan is activated immediately when an accident occurs.

2. Emergency measures: The on-site safety Manager will assess whether the accident will escalate
further according to the type of accident and takes appropriate measures to prevent any further
escalations. Upon confirmation that no harm will be caused to the rescuers, the rescue team will
be directed to enter the scene of the accident to rescue the injured personnel. When the injuries
are serious, to immediately call the local emergency center for assistance.
3.Termination of the emergency plan: The emergency commander can issue an emergency termination
order when the following conditions are met:
i. Emergency rescue operations have been completed and the hazards causing the accident are
effectively controlled and eliminated;

ii. All on-site personnel are accounted for;


iii. There are no other factors that affect the termination of the emergency rescue plan;

4.Resumption of work: After the emergency rescue plan is terminated, effective measures should be undertaken to
prevent further accidents. The site clean-up must be carried out in an organized manner to prevent further property
damage and/or casualties, and protect related physical evidence. After the cleaning is completed, construction and
production can be resumed after approval by the relevant local authorities or departments.

5.Accident summary report: After the conclusion of the rescue, contractors should report the accident
accordingly. Summarize the entire process of emergency rescue plan implementation, improve the deficiencies
and defects in the emergency rescue plan where necessary and provide experience for the establishment,
formulation and modification of future plans.
