RACI DEFINITIONS
The project number is assigned by the PMO (Project Management Office) once the project is approved for funding. RESPONSIBLE The person(s) who does the work. At least one person should
be responsible for each work package.
PROJECT MANAGER ACCOUNTABLE The person ultimately answerable for the successful
completion of the work package. There should be only one
person accountable for each task.
The person responsible for the successful delivery of the project to time costs and quality constraints.
CONSULTED Subject-matter experts whose advice is required.
FULL PROJECT NAME
INFORMATION People who are informed of progress. For example, project
The name or title of the project. admin, or PMO.

NAME OF PERSON OR DEPARTMENT You can change the RACI definitions to meet the needs of your project
For example, you might add Sign-off/Approver.
The name of the person or department involved in the task.
