 The certificate of completion issued by the company/agency after the
training indicating the number of hours completed.

Acknowledgement
 Brief statement in acknowledging people extended their effort and
support for the trainee to complete his training.

Dedication
 Brief statement in dedicating the report and accomplishment made
thru practicum to their love ones.

Chapter 1: The Company


Description / Background
The discussion must focus to the historical background of the
company, when it was founded, who founded, where and when.
Likewise, mention some of the contributions of politicians and
other people who became part of the establishment of the
company. Do not compose this part, content may be available
from the company.

Goal / Philosophy / Mission / Vision / Objectives


State the company’s goal, philosophy, mission, vision or
objectives if there is. If none, skip this part.

Chapter 2: The Office


Description / Background / Function
The discussion must focus to the description of the company, its
function to the organization/company and its transactions.
Organizational Structure
Copy the company’s organizational structure or chart (in figure
format). Or if not available, you can make your own structure
based from its existing organizational hierarchy.
Personnel / Employee Profile
From this part, you just enumerate the list of employees or
personnel from the higher position down to the lowest position of
the office and their qualifications/profile.

Chapter 3: The OJT/Practicum


Work / Job Description
Discuss your work description, how it was assigned to you, who
assigned it to you and how you accepted it.
Work-related Experiences
Discuss everything about your work-related experiences. Work-
related experiences cover the tasks/duties/responsibilities being
given to you every day. How you handle and perform them
accordingly is the main focus of this section.
Peer-related Experiences
Discuss everything about your peer-related experiences. Peer-
related experiences covers the relationship/camaraderie/treatment
of the people towards you every day. How you face and
responded to them accordingly is the main focus of this section.
Problems Encountered
This section will discuss problems you encounter while on
training. Problems could be work-related or peer related issues.
Some inefficient/ineffective performance relative to your work, /
some misunderstanding or behavior related cases can be
consider from this section.
Skills and Knowledge Gained
Discuss everything about the skills/new knowledge you gain from
the training. IT or non-IT related skills or knowledge could be
considered from this section. Just discuss it properly and with
sense.

Chapter 4: Observations and Findings


Company Management
Based from your observation, discuss how the manager/head of
the office handle or manage the office. Management of the office
may mean their commitment to their work, their target
performances and services enhancement towards its clientele.
How they handle risks and challenges behind the call of duty.

Personnel Management
Based from your observation, discuss how the manager/head of
the office handle or manage his personnel/staff. Personnel
management may mean how they handle people, it times of
trouble, in need and support while in the office. How they treat
them accordingly and deal with their work-related
activities/concerns.
Performances and Achievements
Discuss some issues related to the office performance, work-
output, their achievements or reward received for the past two (2)
years. How they attained it and the people behind could be
considered.

Picture with: (Do not stretch, there must be a brief description/caption
from every picture.)

f. DTR (Daily Time Record)
This must be original copy, with signature over printed name of
the immediate supervisor or the HR head. Failure to comply will
invalidate your said document.
g. Evaluation Form
This was issued to you, with an authenticated signature of you
OJT supervisor. Never attempt to alter/fabricate any rating given
to you by your immediate supervisor, failure from the subject is the
least punishment that you can get. Likewise, never lose the
document, there will be no replacement at all.
h. Curriculum Vitae
Just attached a copy similar to the CV you submitted to the
company during application. Must have passport size picture.
