Instruction: Please complete the following, sign and return to the class advisers on or before
January 12, 2024 (Friday).

CONSENT & WAIVER (Please read carefully.)

a) I agree to my child taking part of the Immersion Program as requirement for graduation in the SHS Curriculum
and to expose him/her to the assigned company/units and learn new skills relevant to the theory learned in
the classroom.

b) I understand that my child will undergo an 80-hour Work Immersion at Prime Innova Technical and
Occupational Skills Training L.L.C. with the School Coordinator and In-Company Trainer from
January 22, 2024 to March 7, 2024 on Mondays, Tuesdays and Thursdays as provided in the schedule. I also
understand that this schedule is subject for adjustment if necessary due to unavoidable circumstances.

c) I fully support the Work Immersion undertaking of my son/daughter through financial and others such as but
not limited to my attendance/presence if so desired.

d) I understand that the bus service from school to the immersion venue and vice versa will be provided by the
school. And that my son/daughter must be in school by 7:20 a.m. for the bus service will leave the school
at exactly 7:30 a.m.

e) I will oblige myself to remind my child to abide by the rules and regulations of his/her workplace immersion
venue and will comply with what may be stipulated in a Memorandum of Agreement (MOA). If he/she violates
any rules, regulations, or policies of his/her workplace and of the school that is a valid ground for the
termination of the work immersion therein I understand that he /she may receive a failing grade depending on
the gravity of the offense and after the observance of due process. And that, any amount paid for the duration
of the activity is non-refundable.

f) I will be responsible in reminding my son/daughter to observe and follow all health protocols for his/her safety.

g) I understand further that there are certain risks inherent to the activity. I do hereby assume these risks on
behalf of my son/daughter and waive, release, and hold harmless the partner industry/institution, UIPS LLC
and its Management.
