Declaration :
1. To take supply of electrical energy from the Tata power co. ltd at the aforesaid premises situated within their license area, not exceeding the
sanctioned load of my/our installation as stated above. To be bound by the MERC (Electricity Supply Code and Other conditions of supply)
Regulations, 2005 and as amended from time to time and to provide necessary security deposit as specified therein.

2. To be bound by the provisions of MERC order dated 15th Oct.2009 in Case No. 50 of 2009 pertaining to interim arrangement for Mumbai North area
customer changeover by usage of network infrastructure of Existing Distribution License and any further MERC orders/regulations or otherwise
regarding the same.

3. To be bound by the provisions of the Power Supply Agreement (for all sanctioned load of 50 KW/63 Kva and more) &Substation Space Agreement
(where substation is required) to be executed with Tata Power.

4. To pay for the said supply at the prevailing tariff rates and also to pay the charges based on the Schedule of Charges as approved by MERC for Tata
Power from time to time.

5. That change of name/purpose/classification category shall not be permitted at the time or during the changeover process and the changed-over
consumer shall abide by the conditions of Changeover even after the change of name/purpose/classification category

6. Customers availing supply through AEML agree to pay the necessary wheeling losses/wheeling charges as approved by MERC from time to time.

7. There are no arrears pending against the premises for which I/we have applied for power supply. If any arrears are found to be pending against the
said premises, I/we shall be bound to make payment towards such arrears. Also, I/we have been informed and made aware that if any previous
arrears are pending on the premises, I/we shall not receive any new connection for the said premises.

8. I/we am/are the sole occupant/owner of the above premises for which the above mentioned meters are connected and that the electric supply from
the above connection is used exclusively for the above premises. I also state that I am not receiving supply at the above premises through any other
meter/meters

9. In case of any false representation on my/our part or case of any objection from the present registered consumer or in case the documents produced
by me/us are not absolutely correct. The Tata Power Company Limited reserves the right to transfer the connection from the name of the present
registered consumer.

10. I also under take to keep the Tata Power Company Limited indemnified of any consequences at any time in future in case of any dispute on account of
the transfer of the above connection to my/our/name/names.

11. To pay the Charges for Shifting of Meter/Service (as per MERC approved Schedule of Charges)

12. To pay the amount to be incurred towards the shifting of Services as per the cost estimate, which will be given by Tata Power

13. Permissions for carrying out excavation etc. within consumer premises shall be obtained by the Consumer from the Housing Society/Industrial
Estate/Commercial Complex. All excavations (including obtaining necessary MCGM permit) and back filling required for cable laying/diversion etc.
upto the consumer premises will be carried out by Tata Power.

14. To provide suitable earth pit, in case of shifting or meter panel.

15. To provide suitable weather protection roof for the same in case the Meter panel is mounted on wall outside the building.

16. To undertake removal of excess earth if any and permanent reinstatement of the surface at consumers cost within consumers premises.

17. To uprate the cable and related equipment's after the point of supply to appropriate size to match with the enhanced capacity (for load
extension/reduction/category change/meter shifting)

Important Note: I/We hereby certify that the electricity installation work at premises and the party mentioned above has been carried out by me/us in full conformity with the
prevailing CEA (Measures Relating to Safety and Electric Supply) Regulations, 2010 or any amendments thereof. The particulars of the installation and insulation test result
obtained by me/us are given below.
