Clinical Handout - Week 1 CPR

The document provides instructions for performing cardiopulmonary resuscitation (CPR) and using an automated external defibrillator (AED). It outlines 25 behaviors that are rated on performing CPR safely and effectively, including making the environment safe, checking responsiveness, calling for help, performing chest compressions, giving rescue breaths, using an AED properly, and continuously monitoring the victim.

# Behavior Remarks
1 Make sure that the environment is safe for rescuer and the victim. 3 1 0
2 Ensure that the victim on his/her back on a firm surface. 3 1 0
Initial Assessment Survey / Steps before Starting CPR
3 Within 5 to 10 seconds, check for responsiveness. 3 1 0
[Consider: Unresponsive]
4 Shout for nearby help. 3 1 0
5 Activate the EMS system (997) via mobile device (if appropriate). 3 1 0
Within 5 to 10 seconds, look for no breathing (or only gasping) and check for
6 3 1 0
definite pulse (simultaneously/concurrently).
7 Get a nearby Automated External Defibrillator (AED), or send someone to do so. 3 1 0
[Consider: No Breathing (or Only Gasping) AND No Definite Pulse Felt] to
Perform Full CPR Sequenced Steps (i.e., C-A-B Steps)
The American Heart Association uses the letters C-A-B — compressions, airway, breathing — to help people
remember the order to perform the steps of CPR.
Chest Compression (C)
8 Begin CPR by performing chest compressions by your 2 hands. 3 1 0
Put the heel of one hand on the center of bared chest over the lower half of the
9 breastbone (sternum), and then put the heel of the other hand over the top of the 3 1 0
first.
10 Your shoulder should be right over your hands and your elbows should be straight. 3 1 0
Use your upper body weight (not just your arms) as you push straight down on
(compress) 30 chest compressions with a depth of at least 2 inches (5 cm) for an
average adult should be provided in approximately within 15-18 seconds.
11  Optimal rate of manual chest compressions is between 100/min and 3 1 0
120/min.
 AVOID excessive chest compressions; compression depth should be no
more than 2.4 inches (6 cm).
Allow full recoil of chest after each compression; do not lean on the chest after
12 3 1 0
each compression.
Airway (A)
13 Kneel close to the victim neck or shoulders. 3 1 0
Open the person's airway using the head-tilt, chin-lift maneuver. Put your palm on
the person's forehead and gently tilt the head back. Then with the other hand,
14 3 1 0
gently lift the chin forward to open the airway.
[No head, neck or back injuries are suspected.]
Breathing (B)
With the airway open (using the head-tilt, chin-lift maneuver), pinch the nostrils
15 shut for mouth-to-mouth breathing and cover the person's mouth with yours, 3 1 0
making a seal.

Prepare to give two rescue breaths.

Give the first rescue breath — lasting one second — and watch to see if the chest
rises.
16 3 1 0
 If it does rise, give the second breath - lasting one second.
 If the chest does not rise, repeat the head-tilt, chin-lift maneuver and then
give the second breath - lasting one second.
# Behavior Remarks
Continue Cycles of CPR for Approximately 2 Minuets
Over approximately 2 minutes:
Perform five cycles of (30 compressions: 2 ventilations CPR).
17 3 1 0
- Note: Thirty chest compressions followed by two rescue breaths is
considered one cycle.
Recheck Steps
After approximately two (2) minutes of “compression : ventilation CPR cycles”:
18 [Within 5 to 10 seconds] look for no breathing (or only gasping) and check for 3 1 0
definite pulse (simultaneously).
A. If the victim has no definite pulse and absent (or not normal) breathing,
RESUME CPR (starting with chest compressions).
 Every approximately 2 minutes: Within 5 to 10 seconds, look for no
breathing (or only gasping) and check for definite pulse (simultaneously).
B. If the definite pulse is present BUT breathing is absent or not normal,
CONTINUE artificial breathing only (called rescue breathing).
 Give 1 breath every 5‐6 seconds for about 10‐12 breaths/minute.
19  Each breath should be delivered over 1 second, making the chest rise. 3 1 0
 Every approximately 2 minutes: Within 5 to 10 seconds, look for no
breathing (or only gasping) and check for definite pulse (simultaneously).
C. If the pulse and breathing returned to normal, PLACE the victim in recovery
position, and:
 Continue to monitor the victim.
 Every approximately 2 minutes: Within 5 to 10 seconds, look for no
breathing (or only gasping) and check for definite pulse (simultaneously).
Considerations for Using the AED
As soon as it arrives, stop CPR and turn on the AED.
20 3 1 0
[Consider: The AED is working properly (~ ready for use on victims)]
21 Apply the AED pads on the victim correctly. 3 1 0
Say, "CLEAR!"
22  Ensure that nobody is touching the victim, and the victim’s body does not 3 1 0
touch electrical conductive surfaces.
23 Push the "analyze" button on the AED. 3 1 0
A. If the rhythm is SHOCKABLE:
 Click “Charge” button.
 Push the “shock” or “discharge” button on the AED (to give 1 shock).
 Resume CPR immediately for about 2 minutes (until prompted by AED to
allow rhythm check).
 Continue CPR until advanced life support providers taking over or
24 3 1 0
victim stars to move.
B. If the rhythm is NOT SHOCKABLE:
 Resume CPR immediately for about 2 minutes (until prompted by AED to
allow rhythm check).
 Continue until advanced life support providers taking over or victim
stars to move.
25 Do not remove the AED pads to perform CPR. Leave them in place. 3 1 0
