I. POLICY:

Any member of the Hospital, patients and/or visitor,who has been


diagnosed with (or suspects a diagnosis of) a communicabledisease that
could be spread through routine contact must heed medical advice on
appropriate isolation until theyno longer havethecommunicabledisease or
the communicabledisease is no longer transmissible through ‘routine’
contactor as determined by a medical professional.

Any member of DSMH, who knows that they have a communicable


disease has an obligation to minimize physical contact with other
members of the hospital.

II. PURPOSE:
Data are used to monitor short- and long-term trends, to alert health
professionals to important changes in trends, and to estimate the
magnitude of morbidity and mortality.

III. PROCEDURES:
1. The hospital must abide with the Department of Health Administrative
Order 2008-009 regarding the reporting of infection to public health
agencies.

2. The hospital must maintain a safe, sanitary working condition.

3. From the emergency room, any patient suspected to have diseases that
belong to Category 1 in Administrative Order 2008-009, the admitting
nurse records shall notify Medical Records in charge immediately. Then
the medical records officer will inform the Rural Health Center within 24
hours or by the fastest means possible. The disease or syndrome under
this category includes:

➢ Acute Flaccid Paralysis


➢ Adverse Event Following Immunization
➢ Anthrax
➢ Human Avian Influenza
➢ Measles
➢ Meningococcal Disease
➢ Neonatal Tetanus
➢ Paralysis Shellfish Poisoning
➢ Rabies
➢ Severe Acute Respiratory Syndrome (SARS)
➢ Outbreaks
➢ Cluster of Disease
➢ Unusual Diseases or Threats

4. All cases of notifiable diseases and syndromes should be collated daily by
the Medical Records in charge using case report form. The said report
specifically the (PIDSAR) Philippine Integrated Disease Surveillance and
response should be submitted weekly to the Municipal Rural Health Unit.
The diseases or syndromes under this category includes:

➢ Acute Bloody Diarrhea


➢ Acute Encephalitis Syndrome
➢ Acute Hemorrhagic Fever Syndrome
➢ Acute Viral Hepatitis
➢ Bacterial Meningitis
➢ Cholera
➢ Dengue
➢ Diphtheria
➢ Influenza-like illness
➢ Leptospirosis
➢ Malaria
➢ Non-Neonatal Tetanus
➢ Pertussis
➢ Typhoid and Paratyphoid Fever
