Policies Reporting Notifiable Diseases
I. POLICY:
II. PURPOSE:
Data are used to monitor short- and long-term trends, to alert health
professionals to important changes in trends, and to estimate the
magnitude of morbidity and mortality.
III. PROCEDURES:
1. The hospital must abide with the Department of Health Administrative
Order 2008-009 regarding the reporting of infection to public health
agencies.

3. From the emergency room, any patient suspected to have diseases that
belong to Category 1 in Administrative Order 2008-009, the admitting
nurse records shall notify Medical Records in charge immediately. Then
the medical records officer will inform the Rural Health Center within 24
hours or by the fastest means possible. The disease or syndrome under
this category includes:

5. Second copies are to be kept in the medical records section for safe
keeping and for future references. Based on the Administrative Order, the

6. The outcome of the collated data will also be reported to the infection
Control Officer. Activities are then performed to prevent the spread and
outbreak of the disease.
