Testing Your Thoughts

This worksheet is a simplified version of the Dysfunctional Thought Record meant for clients like adolescents. It guides users through identifying a situation and their thoughts/feelings about it. It then prompts examining what evidence supports or counters the automatic thought and exploring alternative perspectives, possible outcomes, and advice for a friend in the same situation. The goal is to test and potentially change unhelpful thoughts.
