INTRODUCTION

Appendicitis occurs when appendix ( small,worm like ,tubular


appendage attached to cecum of colon) becomes blocked and
becteria invade and infect wall of appendix .
Most common complication of appendicitis are rupture,abscess
and peritonitis.

DEFINITION

• It is an acute inflammation of vermiform appendix (a small finger like appendage attached to


cecum just below the ileocaecal )

CAUSES

• Infection
• Obstruction of faecal matter
• Inflammation and fibrosis of colon
• Most common in emergency abdominal surgery
• Lymphoid hyperphagia.

SIGN AND SYMPTOM


• Acute abdominal pain ( 6-12 hrs)
• Anorexia
• Nausea/vomiting
• Pain at most common Burney’s point
• Fever ( 101’ c)
• Constipation
• Rovsign’s sign
• Abdominal distension
• Local tenderness with pressure
PATHOPHSIOLOGY

Due to infection or faecal stone and obstruction


Obstruction in appendix lumen.
Increase intra-luminal pressure
Disturb blood supply
Edema or swelling in appendix

Rupture of appendix
DIAGNOSTIC EVALUATION

• Blood studies –WBC increases


• Laparoscopy
• Urinalysis
• By positive sign and symptoms.
MANAGEMENT

Ø Administer antibiotic and iv fluids until surgery.


Ø Analgesics agent can be given after diagnosis is made.
Ø Ice bag therapy.
Ø Comfortable position.
SURGICAL MANAGEMENT

Ø Appendectomy (Removal of appendix )


NURSING MANAGEMENT

Ø Preoperative care ( before surgery )


Ø Intraoperative care ( during surgery or procedure ).
Ø Post operative care ( after surgery ).
