4. Pending submission of all the above, my child/ward shall be considered as temporary


enrolled.

5. Agree that the official record of my child/ward from this school shall only be released
upon the submission of the transfer credentials from the previous school.

6. The school shall only issue a temporary progress report card signed by the adviser to
monitor the progress of my child and that it is inadmissible for transfer and enrollment
purposes. Moreover should my child attain the qualifying average and other criteria for
academic honors, he/she will not be recognized pending submission of all documents
required;

7. My child can neither be officially promoted to a higher grade level nor officially graduate
from this school.

8. I allow Arellano University (AU) to process and/or disclose to third persons as may be
required, the information including personal information herein provided as defined
under the Data Privacy Act of 2012.

9. Finally I declare that I shall hold free from any liability, whether civil, criminal or
administrative, Arellano University Officers and/or Personnel who are involved in the
acceptance and enrollment of my child, and the enforcement of any law or rule and the
obligations provided in this document.
