JSA Loading and Unloading Items Using Forklift

This document provides a job safety analysis for loading and unloading items with a forklift. It identifies risks such as untrained operators, improper lifting equipment, carrying loads too fast or high, and operating on slopes. To control risks, it specifies that only trained operators can drive forklifts, forklifts cannot carry passengers, speed must not exceed 5 km/hr, forklifts must be well-maintained and inspected, loads should not overload capacity, and loads must be carried slowly and at a low height. Signal persons should also be used when visibility is blocked.
