1. This pertain to the incident transpired at Car Park Entrance Shaw last November 11, 2023
(Saturday) that around 1440H where in the Contractor of Mr. Samson Tan under Unit 31-V are properly
on hold at Car Park Entrance Shaw for proper screening.

Summary
1. At around 1440H In House contractor of Unit 31-V named Mr. Joselito Bayta, Mr. Benedicto
Bayta and Mr. Michael Abel arrived at the area on board Service Vehicle L300 with Plate# PIK-688.
2. On duty Security Personnel Sg John Carlo Pacibe properly ask the workers if what Unit they will
go, the worker said “ HINDI PO NAMIN ALAM SIR KUNG ANONG UNIT”.
3. Security on duty Sg Pacibe properly adviced the workers to call there contact person
(Tenant/Unit owner) as a proper Security Operation Procedure. (SOP)
4. After so many attemp of calling to their contact person the worker said “SIR WALA PO
SUMASAGOT SA TAWAG KO” so being the Security Procedure the workers are on hold for us to verify
what Unit they will work.
5. After a few minutes of waiting Security on duty Sg Pacibe ask the worker if who is their contact
person here at Zitan Tower, the worker said its Atty. Tan.
6. As Sg Pacibe knew that Atty. Tan was the Unit Owner of 31-V so he called CCTV Operator
seeking assistance to call the receptionist on duty Ms. Janice Camungay to informed Atty. Samson Tan
that his workers are positive at Car Park Entrance Shaw.
7. The undersigned Gathered information from Receptionist on duty Ms. Janice Camungay as she
called Atty. Tan via intercom at Unit 31-V asper the receptionist Atty. Tan is very angry, then he came
down at the reception asking why his contractor are not allowed to enter.

Action Taken:
1. This duty Officer conduct initial investigation about the concern transfired last November 11,
2023 at Car Park Entrance Shaw area.
2. Properly informed the Property Officer about the concern.
Recommendation
1. There is a work permit but the contractor don’t have knowledge if what Unit they are going to, With
this matter the undersigned highly recommended that all Tenant/Unit Owner should give their Unit
number to the contractor for faster screening of duty Security Guard.
2. Upon received the Gate Pass/Work Permit be sure that they must have them write clearly specially
date and Unit number before the guard accept it.
3. The undersigned recommended that Sg John Carlo Pacibe will be given diciplinary action of our
agency with include his previous violation stated.
A. Last November 6, 2022 Failure to informed duty Officer where in the workers were able to
work even with out a work permit.
B. . Last February 12, 2023 Abandoned his post at perimeter area with out informing the duty
Officer.
C. Involved in fist fighting inside the premises.
4. This is to informed your good office that only the client and the 1 st Eagle Head Quarters are the
authorized to issued diciplinary action.
