Case Note Activity


This activity will be done in pairs. One partner will take the role of an intake worker who
is working in a youth homeless shelter. The second partner will assume the role of the
youth who is attempting to access services at the shelter. The youth in this scenario is
19 years old and has been homeless for the past 6 weeks. They have been couch
surfing and on the odd occasion sleeping in bus shelters. The youth does not appear to
have appropriate winter clothing. It has become too cold for this person to sleep outside
anymore and no one is able to put them up for the night. The youth has been able to
continue to attend school at Conestoga College and maintains a part time job at a local
clothing store. They have been making use of local meal programs and food banks.
Going home to family is not an option at this time as they left 4 years ago as the home
situation became abusive and they have not had any contact with any family members
since leaving.

Throughout this process the person acting as the shelter staff will be asked to conduct a
thorough assessment of the youth’s needs and summarize the interview in a case
note using the case note guidelines as presented in class. Week 4 Power Point.
Follow the guidelines below to ensure you have conducted a thorough intake interview.
Program Intake Assessment Work Sheet
1. Introduce yourself to the client and explain your role at the Agency
a. Child and Youth Worker
b. Supervisor
c. Recreation coordinator
d. Concurrent Disorders Counsellor
e. Part-Time Counsellor
f. Etc.

2. Explain the program to the youth


a. This is a 20 bed youth shelter servicing males and females 15-24 years of
age etc. etc. etc.

3. Ask the youth if they have any immediate needs. Ensure the following are
covered: (THIS MUST BE IN YOUR CASE NOTE)
a. Food
b. Medical
c. Shelter

4. Once you have satisfied the immediate needs, conduct an intake

Are you currently taking any medications? Do you have any medications with you? (It
would be important to document the medication the youth has and to inform them that
all medication must be kept locked in the staff office and administered by staff until
discharge)

After completing the interview, complete your case note and indicate what the result of
the intake interview was. Did the youth choose to be admitted? Did they choose to
leave? If they chose to leave, did they at least inform us of where they were going? If
they choose to stay, indicate this in the case note.

Marking Guidelines:
Complete = 1 Point
Detailed = 3 Points
Spelling and Grammar = 1 Point
Total Marks – 5
