b. Compliance to Referral Codes

Building shall conform to the provisions of the National Building Code of the Philippines
(PD 1096). Accessibility Law (BP 344), National Structural Code of the Phitippines,
Plumbing Code (RA 1378, 1993-1994 Revisions), Fire Code (RA 9514), Phitippines
Electrical Code and other laws and regulations covering environmental concerns and
local ordinances and regulations.

lv. GENERAL SCOPE OF WORK

The Bidder, by submitting his Proposal, represents that:

a. He has thoroughly read or examined carefully and understands fully all the
proposal documents and his proposal will be in accordance therewith.

b. His Proposal is based upon the conditions and requirements of the proposal
documents without exception.

c. He / She has visited and inspected the Site of Works and its surroundings
and has determined for and satisfied himself as to all matters pertaining to the
project, including the location and the nature of the work; climatic conditions; the
nature and condition of the terrain: geological conditions at the site;
transportation and communication facilities; the requirement and the availability
of materials, labor, water, electric power and roads: the locations and extent of
aggregate sources, and other factors that may affect the cost, duration and
execution of the work; that he has determined the general characteristics of the
project and the conditions indicated above.

d. He / She has acquainted and familiarized himself with all conditions, local or
otherwise, affecting the car'rying out of the contract work and has arrived at an
estimate of the facilities available and the facilities needed for the project.

e. He / She has familiarized himself with all laws, decrees, regulations of the
Philippines and the Local Government, which may affect or apply to the
operations and activities of the contractors.

f. He I She has conducted an actual site survey of the project area preliminary
identifying the limitations of land utilization. ln the process, he shall be able to
familiarize himself with the site and its surrounding.

g. He / She has determined existing infrastructure, facilities, utilities, etc., which


may have a bearing on the planning and design consideration; the bidder shall
identify/locate the existing utilities at the site, namely:
Electrical Power Supply
Water Supply
Sewer and Storm Drainage
Telephone Lines
Others that may be necessary

h. He / She is aware that the contract duration of the project shall be 90 days.

Construction Phase

c. Supply and installation of 1.5mm Lead lining of walls at the
examination room, up to B ft. from FFL.
d. Design, supply and installation of lead lined doors complete
with hardware and accessories
e. Design, supply and installation of hollow core flush door
complete with hardware and accessories
f. Supply and installation of lead lined viewing window frame
and 100cm x 200cm x 7mm Lead Glass
g. Supply and installation of ceiling rail steel frames
h. Supply and installation of cable bracket for C-arm
i. Supply and installation of fixed gypsum board ceiling with light
metal frames
j. Supply and installation of anti-static vinyl sheet complete with
20cm cove with capping
k. Painting of walls in the examination, technical and control
rooms using anti-bacterial paint
l. Painting of doors of examination, technical and control rooms
m. Painting of ceiling using anti-bacterial paint
n. Restoration and repairs on any affected Areas

4. Sanitary/Plumbing Works
a. Checking and relocation of existing pipings that may clash
with infra works and renovation
b. Provide Waterproofing (if needed)

5. Electrical Works
a. Supply and installation of ECB tapped to existing 460V
busbar inside hospital electrical room at 2nd floor for
equipment power supply
b. Supply and lnstallation of Appropriate transformer for
machine and utilities
c. Supply and installation of ECB tapped to existing 400V bus
bar inside hospital electrical room at 2nd floor for equipment
power supply
d. Supply and installation of TVSS
e. Supply and installation of ECB tapped to existing 230V panel
inside hospital electrical room at 2nd floor for air-conditioning
units
t. Supply and installation of appropriate 460V, 400V and 230V
electrical feeder line
g. Supply and installation of UPS breaker at technical room
h. Supply and installation of EPB for equipment complete with
controls
i. Supply and installation of power cables from UpS breaker to
Cath lab machine

a. Supply and installation of air-conditioning unit in the
examination, technical and control room rooms
b. Supply and installation of laminar air flow system complete
with appropriate duct works and accessories for examination
room
c. Supply and installation of vacuum outlet (suction ouflet)
d. Relocation of medical gas outlets
e. Testing and Commissioning

7. Others
a. Catheter Cabinet with glass door inside examination room
b. Additional Cabinets and counter in the Control room
As a rule, contract implementation for procurement of infrastructure projects shall comply
with Annex "E" of RA 9184. The following provisions shall supplement these procedures:

A. The contractor shall submit a detailed program of works, S-Curve, PERT CPM or
Master Schedule within fourteen (14) calendar days after the issuance of the Notice
to Proceed for approval by the procuring entity that shall include, but will not be
limited to:
1. The order in which it intends to carry out the work including anticipated
timing for each stage of detailed planning and construction;
2. Periods for review of specific outputs and any other submissions and
approvals;
3. Sequence of timing for inspection and tests;
4. General description of the design and construction methods to be adopted,
5. Number and names of personnel to be assigned for each stage of the work;
6. List of equipment required on site for each stage of the work; and
7. Description of the quality control system to be utilized for the project.
B. The Contractor, Project Manager, and LCP shall schedule a Kick Off Meeting before
the Construction Day 1 to set construction prerequisites, deliverables, clear and
approved Master Schedule of the project signed by all parties.

C. Any error, omission, inconsistencies, inadequacies or failure submitted by the


contractor that do not comply with the requirements shall be rectified, re-submitted
and reviewed at the contractor's cost. lf the contractor wishes to modify a design or
document which has been previously submitted, reviewed and approved, the
contractor shall notify LCP within a reasonable period of time and shall shoulder the
cost of such changes.

D. The contract documents shall include the manner and schedule of payment
specifying the estimated contract amount and installments in which the contract will
be paid.

E. The contractor shall be entitled to advance payment subject to the provisions of


Section 4 of Annex "E", IRR (RA 9184).

F. The contractor shall provide Comprehensive Assessment Reporl and Construction


Methodologies detailing the required modifications and upgrades to the existing
operation room as well as all necessary equipment, personnel, instruments,
documents and others to carry out specific tests.

G. This project shall have a maximum Defects Liability Period of one (1) year after
contract completion or as provided for in the contract documents. This is without
prejudice to the liabilities imposed upon the engineer/architect who drew up the
plans and specification for the building as sanctioned under Section 1723 of lhe
New Civil Code of the Philippines.

H. Warranty against Structural Defects and failures shall conform to the provisions of
Section 62.2.3.2. of RA 9184.

v. ELIG!BILITY REQUIREMENTS

A. Basic

1. The eligibility requirements for the project shall comply with all provisions
of Sections 23-24 of IRR of RA 9184.

2. The contractor must have a minimum of 5 years of experience in


managing and completing complex healthcare facility construction projects,
specifically operating rooms and/or radiology imaging systems

3. The contractor must employ a team of highly skilled professionals,


including architects, engineers, and technicians, experienced in structural
retrofitting and constructing operating rooms and/or radiology imaging systems

4. The Contractor must have completed a similar single contract project in


the amount of at least fifty percent (50%) of the ABC computed within 5 years.

5. The Contractor must have minimum PCAB License Category D

6. lf the Contractor had previously done projects within the lnstitution or


other agency, Contractor must have a Certificate of Satisfactory Performance
given by the agency.
A. Professionals Eligibility

1. For the Construction Team, the Bidder must assign to the project
professionals as shown below:

a) Project Manager (1)


o Licensed Engineer or Architect
o At least 5 years of experience in construction management
o Good oral and written communication skills
r Must have experience in structural retrofitting of a hospital
b) Project Engineer (1)
o Licensed Structural Engineer
r At least 5 years of experience in O.R. / hospital construction

o Must have experience in structural retrofitting of a hospital

o At least 5 years of experience in O.R. / hospital construction

o Must have experience in structural retrofitting of a hospital

VI. BIDDING REQUIREMENTS

A. Proposals
lnterested bidders shall submit two (2) sets of the following:

VII. APPROVED BUDGET COST

The total approved budget cost for the Project is php 25,000,000.00

VIII. TIME FRAME

The Contractor is required to complete the Project within g0 Calendar Days or as


shown below, to start upon the Contractor's receipt and signing of Notice to Proceed.

IX. OBLIGATIONS AND LIABILITIES

A. To comply with the requirement as set forth in the LCP bidding documents as provided for
on RA 9184 and its IRR and other applicable rules and regulations related to the project.
B. To conduct site inspection before participating the bidding to consider all conditions that
may directly or indirectly affect the implementation of the project, including verification of
measuremenUdimensions of the plans/drawings.
C. To provide and guarantee the highest quality of workmanshlp. All works must comply with
the standard, approved plans, scope of works and technical specifications provided for by
LCP. Non-complied or non-acceptable works must be corrected without cost to LCP.
D. Retrofitting works must be in compliance with the engineering standards of the National
Structural Code of the Philippines, and D.O. No. 88 Series 0f 2019 of Department of Public
Works and Highways.
E. Provide the following on his own accounts/expense;
1. Suitable Staging, temporary office at specified location inside the LCP grounds for his
workmen.
2. Suitable and approved fences/barricades around the project working area to safeguard
his workmen and the public against accidents.
3. Rigid covers equipped with negative pressure / exhaust to safeguard the equipments to
dust, and damage.
4. Proper Personal Protective Equipment (PPE), uniform and first aid kits for its workmen
while inside LCP premises.
5. Record and logbook for daily attendance of its workmen and activities.
F. The contractor will be liable to any damage to LCP property that may occur during the
construction period.
G. Any equipment or machine that may be damaged or malfunctioned during the construction
duration will be charged to the contractor.
H. To provide licensed Engineer & Architect that will constantly coordinate with LCP authorized
representative to decide on normal and critical condition during the construction phase.
There should be a weekly meeting (or more often when necessary)for both parties to
discuss the progress and other matters related to the project.
l. To seek approval, at all times, from LCP Representative regarding tapping of water,
sanitary, and electrical works.

J, To submit complete sets of as-built plan, requirement for the release of final payment.

K. To submit detailed shop drawings, detailed estimate and adjusted work schedule in any
additional works, change order/variation order. Shop drawing shall be signed and sealed.
lmplementation shall be subject to verification and recommendation of Engineering Section
and shall be approved by the LCP.

L. To comply with LCP's standard operating procedures, policies and regulations, such as but
not limited to:

1. All deliveries of materials must pass through LCP Materials Management Division -
Receiving Area, duly supported by a delivery receipUsales invoice. LCP Engineering
shall check conformity of specifications before acceptance.

2. All tools and equipment to be brought-in must pass through LCP Security Office for
issuance of entry pass. Pull out of tools and equipment must be with corresponding gate
pass issued by the Property and Supply Section.

3. Tools and materials must be delivered 100% to finish the project as per plans and
specifications. All excess scrap materials will become property of LCP.

4. Safekeeping and safeguarding of tools, equipment and materials shall be the


accountability of the Contractor.

5. Avoid any acUs that will cause disruption of hospital operation. The contractor shall be
held liable for all damages incurred during construction. Restoration of damages shall at
all their own expense.

6, Entry and exit of workmen is subject for inspection by LCP guard.

7. Secure work permit at engineering section before commencement of work.


8. Policies and regulations reflected on approved work permit shall be complied at all time.
H. LCP will be conducting Constructors Performance Evaluation System (CPES) on this
project and will require the followrng documents during construction:

a) Contract documents: approved contract agreement, contract drawing,


general and special/local provisions including plans and specifications, method of
construction, safety and health measures.

b) Approved constructor's construction program: CPM work schedule, or bar


cha11, materialsimanpower schedule equipment schedule. and organizational
char1, financral chart such as S-curve or cash flow.

c) Constructor's statistical reports: physical progress/status. equrpment and


manpower schedules based on the latest approved PERT/CPl\4.
d) Materials testing procedures, records of tests and results. materials quality
control program.

e) Constructor's operational set-up. general lay-out of facilities. project office


and q uarters. warehouse. shops for repair/mai ntenanceicarpentry/rebar.

s) Punch list (for final visit upon completion).

h) Relevant checklists, CPES evaluation form and cover sheet.

X. NON.DISCLOSURE AGREEMENT

The winning bidder/supplier shall protect Confidential lnformation about the LCP, and to use it only
for purposes of complying with its tasks and obligations under the Official Bid
DocumenUContract/Purchase Order. lt shall not disclose any Confidential lnformation about the
LCP project or procurement to a third party without the consent of the LCp.

"Confidential lnformation" refers to information concerning the affairs of the LCP which are not
available to the public which, if disclosed, may compromise the security of the institution.
The winning bidder/supplier shall not disclose any Confidential lnformation acquired during its
engagement with LCP even after the expiration or termination of the Contract.

All questions in dispute this agreement shall be submitted to Arbitration in accordance with the
provisions of the Philippines Law on Arbitration, Article 2042 of the New Civil Code of the
Philippines, and Republic Act No. 876.
