We, Shaira . Adam and Glyndee Marr E. Jagonob, third-year nursing students of Cotabato
Medical Foundation College, Inc., are conducting a study entitled “Effectiveness of Herbal and
Prescribed Medicine in managing patients with hypertension” to comply with the requirements
for the degree of Bachelor of Science in Nursing. The study aims to compare the effectiveness of
herbal and prescribed medicine in managing patient with hypertension.
In connection with this, we would like to request permission to allow us to conduct a pilot test
for the 10 indigenous selected participants who are diagnosed with hypertension. This is to test
the reliability of our research instrument. Rest assured that whatever information gathered will be
kept confidential and be used for research purposes only.
