5. Gather equipment and supplies; place on a clean work 1
surface.
6. Don clean snug fit gloves 1
7. Place patient in comfortable position. Apply tourniquet and 1
assess potential insertion sites.
8. If necessary, encourage venous filling by tapping the vein 0.5
lightly, gently opening and closing the fist, lowering the
extremity below heart level,
9. Cleanse intended site with antimicrobial solution. Allow to 1
dry. Do not re-palpate the vein or touch the skin.
10. Remove the safety cannula/device from the packaging and 1
inspect for any faults
11. Stabilize vein and apply traction on skin, above the 1
intended site with non-dominant hand
12. Insert catheter bevel up at 5 – 15 degree angle. Observe for 1
blood return within flashback chamber.
13. Holding stylet steady, push catheter off stylet and into vein 1
until catheter hub is situated against the skin.
14. Release tourniquet. Remove stylet. Engage safety 1
mechanism as necessary.
15. Secure to skin with tape 0.5
16. Flush catheter while observing for any complications. 1
17. Apply transparent dressing. 1
18. Label IV site with date, initials, gauge, and length of 1
catheter.
19. Dispose of used supplies per policy.. 1
20. Talk to patient & thank him for cooperation 1
21. Wash hands 1
22. Document the procedure in patients record & Nurses notes 1

*Note for examiner:
Practitioners should only make two attempts at cannulation.
If both attempts fail the procedure should be referred to another experienced
colleague.
Cannulas are single use only and a new cannula must be used for each attempt.
Each correct step carries “1” Mark & each missed step ot incomplete step carries
“0” mark
