Part 1 — Basics
Here you’ll gather basic information, to help you to measure changes and improvements as you work
with The Body Code. Be as accurate as possible. Complete the form entirely for each session if desired.

Issues and weekly or monthly experience. A one is most minor, a ten is most
severe. Using this pain scale can help to identify the apparent
Discomfort severity of the issue(s). You’ll gather these same pieces of
information again later on after doing some Body Code work, to
Evaluation measure any changes and/or improvement.
