We, the BEED students at the Polytechnic University of the Philippines-Cabiao


Campus, are respectfully requesting your good office for your permission to conduct a study
titled “English Language Learning Anxiety in Junior High School Its Effect on
Communicative Performance” at Cabiao National High School using a survey
questionnaire.

In fulfillment of this study, we kindly request your permission to distribute the survey
questionnaire to the students during school hours on June 7, 2023 8 am-12 pm to the Grade
7 students. We assure you that the survey administration will not cause any disruption to the
regular school activities.

If approved, rest assured that the data of this study will remain confidential and will
be used for educational purposes only. Thank you for considering our request We are
