Our Dear (name and position of DepEd highest official present) I have the honor to present to you the
(total no of candidates) candidates for moving up/graduation of whom (number of boys/male) are
boys/male and (number of girls/female) are girls/female of for SY 2021-2022.
They have satisfactorily completed all the requirements prescribed by the Department of Education
under the K to 12 Basic Education Program for Kimdergarten/Elementary/JHS/SHS Curriculum. May I
now request (name and position of next higher official present) to certify their eligibility for graduation.

In my capacity as (Position Title), after careful scrutiny of the requirements submitted in my office and
found that you have completed the requirements for moving up/graduation, I hereby certify that you are
eligible for moving up/graduation having completed satisfactorily the requirements for
Kindergarten/Elementary/JHS/SHS curriculum prescribed by the Department of Education under the K
to 12 Basic Education Program.
May I, therefore request (name and position of DepEd highest official present) to confirm their
graduation.
