P.O, District and State) do hereby solemnly affirm and declare as follows:-

1. That I am the owner of the ________(premise address)_________ and I have given


this premise on rent to Mr.___tenant name______, Son/W/D of __________ on rental
basis by executing a proper legal rental agreement.

2. That the rent of the abovesaid premise is Rs. 20,000 (twenty thousand) only per
month as decided by us through the valid rent agreement.

3. That the tenant(name) has deposited 1,30,000 (One lac Thirty Thousand) as security
amount in the presence of witnesses to be adjusted as against the arears of rent if
necessary.

4. That he could not pay the rent for consecutively 3 months i.e., May, June, July. And
consequently rent for these period (Rs. 60,000) was adjusted from the deposit amount as
per the clause mentioned in the rent agreement.

5. That he agrees to stay as tenant in the abovementioned premise for further period of 3
months and fifteen days starting from 10 th August 2023 till 25 November 2023 and the
rent for the same will be adjusted from security deposit i.e., 60,000 for three months and
10,000 for further 15 days.

6. That he undertake to remove all items/material and vacate the premises and hand
over possession to (Name of the owner) after the said date, unconditionally.

7. That he agrees to adjust remaining balance amount of Rs. 30,000 of security deposit
as against any damages, repairing and electronic charges

8. The statements made above are true to the best of my knowledge and I sign this
