made any progress since the last time, or if you are still stuck at this point, and be able to determine
how you should proceed from here.

C. CHECK (15 mins)

REFLECT: Activity 4: My MISSION Statement

Instructions: Fill-in the blanks with your own words to complete your mission statement. This
will serves as a promise and reminder for yourself to cultivate a healthy and positive mindset.

I, Chayne Althea M. Requina, promise to live a fulfilling, successful, and joyous life. The road
may be bumpy, and the future unforeseen, but I will make my dreams come true.
I want the people in my life, those who matter, to see kindness, resilience, and authenticity in me. But I need
to be okay with the fact that not everyone will see these in me because the people who are important are the
ones who love me for me. And I need to be okay with the fact that I am not perfect, but the best I can do is try.

My goals are personal growth, academic success, and fostering meaningful relationships. I will dedicate
time and effort to self-improvement, study diligently, and communicate openly and sincerely with
those around me to reach my goals and become the best version of myself.
