ABSTRACT

The Mislang clinic Application is a web- based A digital keeping record service system

where the doctor and medical assistant can easily find the previous record of the patient, while

the patients can potentially increase to avoid long queues and prevent infection in others

sickness people to their check-up, in this web with a medical assistance have more convenient

way to find files of the patient. This web application is an alternative solution for the existing

issues that arises in the traditional manual medical system and because of looking for paper,

this is also one of the ways in which checking up takes time process such as the medical

assistant difficulty in finding a papers critical hours and downtime caused by the due to finding

old papers or if they have been lost and thrown away around, while finding for a patient it

takes a long time. The proponents used the Rapid Action Model as the guide in the

development process of the research and the planned steps are Planning, where the

proponents conducted research and analysis for the current system in the transportation

industry. Design and Coding in which the proponents initialized the design model for the

system and conducted the actual coding and programming. The last step is the

Implementation, where the proponents conducted a simulation process in which the system is
tested and utilized to confirm and assure the functionality along with the survey evaluation

which resulted to an over-all average of 3.88 with the verbal interpretation of Strongly Agree.

In the development of the web application, the proponents use Visual Studio Code C# with SQL

Server.

ABSTRACT

A digital student assessment and progress monitoring system in senior high


school for Santo Tomas, Batangas, is a system that can monitor student performance on
academic tasks. In this system, the teacher can upload the lesson that students need to
study. The student can get the lesson and pass their activity easily by accessing the
system, so the teacher doesn’t have to struggle to collect the students' performance
tasks because they can both use the system. The student's parents have access to the
system, and they can monitor their child transparently anytime and anywhere. The
performance of both teacher and student in terms of education will increase with the
help of the system. Parents will be satisfied with the way their child is being monitored
because they are paying attention to the student's studies while working. The researcher
used the agile model as a guide in the developmental process of the research, and the
planned steps were: Requirements, where the project was planned and the data
gathering for the same information was to be analyzed before it started; Design, where
the research was trying to get the design that the client needed and also to visualize it;
Development, where the researcher starts to execute the design model for the system
and conduct the actual coding and programming; Testing, where they will test the
system's appearance, capability, and functionality; Deployment, where the system is
finished and ready for deployment; and, at the last stage, Review, where the review is
the feedback, suggestions, and satisfaction of the user that were given to the
programmer. In the development process, the researcher used Sublime Text 3 to build
the digital monitoring system in the languages of CSS, PHP, and SQL. The researcher

used Xampp, an online database system that allows for sufficient data management,
access control, and security, to manage the system's database.

CHAPTER 1

INTRODUCTION

Education is the most important aspect that an individual need to develop towards

achieving a better future. Every educational institution should focus on improving

students' knowledge, skills, and competences. The different learning modalities meant

for the distance learning the school may adapt depending on local health condition. this

includes face to face, distance learning, blended learning and homeschooling. All

teachers are challenged by teaching students in the twenty-first century focusing

particularly on how certain procedures and knowledge will be imbedded in and

assimilated by the students. On the other hand, according to the study of C.B Agaton et

al (2021), parents face various challenges from distance learning in terms of the virtual

setting; delivery of instruction; unsatisfactory learning outcomes; struggle with the use

and availability of technology; personal problems on health, stress, and learning style of

their child; as well as financial difficulties while working for the family during lockdown.

These findings serve as a basis for providing a comprehensive and inclusive education

policies while considering the perspective of the learners’ parents during the pandemic

and beyond. The COVID-19 pandemic causes substantial impact on students, teachers,

and the parents as well.

Furthermore, remote learning is the widely held choice for ensuring education

continuity during the pandemic. Remote learning takes place when a student and

teacher or source of information are separated by distance or time. It can be done

through synchronous (happening at the same time) or asynchronous sessions. It includes

learning kits delivered to homes, TV broadcast, radio programs, WIFI-connected devices

and micro-content deliveries. The benefits associated with virtual schooling are

expanding educational access, providing high-quality learning opportunities and

improving student outcomes and skills. However, the research to support these

conjectures is limited at best. The only students typically successful in online learning

environments are those who have independent orientations towards learning and are

highly motivated by intrinsic sources. M.Barbour et al (2009).

On the other hand, the distance education provides individuals with educational

environments that are not dependent on their location, time, or space, with the support

of advancing digital platforms and technologies. Bilgiç H. G., et al (2015). Social media

may be used to get around some of the educational obstacles that both students and

teachers now face. The instructor and student will properly transmit knowledge

between themselves. Also, social media platforms will make it simpler for teachers to

gather information about their students' activities, but there are so many of them that it

could be difficult for them to decide which one to use. Giving the student a nutritious

education will create a better future for this nation. The development of everyone's life

depends on education. Everyone shares the same value in it. It is everyone's right to be

educated.

Without education, life can become extremely difficult and difficult to survive.

1.1 Background of the Study

The school of Santo Tomas Senior High School is located in Barangay San Miguel,

City of Santo Tomas, Batangas, and is under the respectful leadership of Principal II, Dr.

Gigi C. Corpus. The population of students in said school is approximately 2000. The

school is built to serve the student who wants to learn and also the student who has a

vision for her or his future. The teachers are delivering the proper education and

ensuring the quality of it to the students under the supervision of the principal.

Moreover, both private and public schools are affected and must abide on the

new orders of delivering education in the new normal, it was observed by most schools

that led to make ways in making the activities and programs accessible and lighter for

learners even in a distance learning. One of the steps made is the digital student

assessment and progress monitoring system in senior high school for Santo Tomas,

Batangas where transmit the basic understanding, capabilities, and values which are

needed. Monitoring system also offers a service in accordance with the requirements

link video lesson, teacher made test will be uploaded and set in the system. The said

system online and usable that learner can access and use the system.

1.2 Statement of the Study

The general problem with this study is that the educational needs of every

student are not met correctly. The study aims to create a monitoring system that will

provide and deliver the proper education that every student needs.

Specific Problems:

• Collecting student activities is difficult for the teacher because they don't know

which platform the students are using for passing the activity.

• The student parents were busy at work, so they didn’t monitor their child's

academic progress.

• Students faced academic challenges such as a lack of information, infrastructure,

school materials, and teachers.

1.3 Objective of the Study

The study aims to create and design the Digital Student Assessment and Progress

Monitoring System in Senior High School for Santo Tomas, Batangas that would enable

the teacher and parents to monitor the student's progress transparently. To establish a

trustworthy and beneficial system for Teachers, Students, and Parents at Santo Tomas

Senior High School.

The following statements are the specific objectives of this study:

• To create and design a platform that will allow teachers to insert all of their

lessons and also help the teacher to distribute the activities of each student.

• To create and design a system that would help teachers collect and compile

student activities.

• To create and design a monitoring system that will give satisfaction to the

parents to monitor their children.

• To create and design a user-friendly system that will help the students with their

academics and deliver the proper education that they need.

1.4 Scope of the Study

• The teacher can insert their lesson and record the student activities using this

digital monitoring system.

• The student can upload the softcopy activity and view the student's lessons

easily in the system.

• Parents' involvement in the student's academic progress will be transparent. 

The registered teacher, parent, or student can only access the system with the

supervision of the admin.

1.5 Limitation of the Study

• Student attendance is not included in the digital system.

• Exams and quizzes are not included on the digital system.

• SMS notifications for updates on student progress are not included.

1.6 Significant of the Study

The outcome of the study will be the definition of the Digital Student Assessment

and Progress Monitoring in Senior High School for Santo Tomas, Batangas. The findings

of this study will be great help of the following:

A teacher can use this digital monitoring system for educational purposes like

uploading and posting lessons. This type of system will provide a platform for delivering

the proper meaning of education to student. However, this system was designed to help

teachers avoid overthinking their students' academics and also to monitor students'

A parent can monitor the academic progress of their student by using this digital

monitoring system. This system will help parents avoid having to go to school because

some of them are busy at work and they need to file a leave to ask only the teachers

about the academic progress of their children.

Student can study the lessons and upload the softcopy activity. This type of

system will provide them information about their academic matters that will be greatly

helpful for their education.

The current researchers will use the research as a guideline and a reference to

develop their research literary skills.

For future researchers, to be serve as their guide, and reference to those who

will conduct similar study. It can also help to widen their ideas and put into practice

what they have learned theoretically.

1.7 Definition of terms

1.7.1 Operational Terms

Academic- Activities, seatwork, and project involve studying and reasoning.

Academic Progress- it is the student’s progress towards successful completion of

the academic requirements of the Course in which they are enrolled. Distance

Learning- the correspondence courses, in which students would communicate

with their schools or teachers by mail.

Intrinsic- belonging to the essential nature or constitution of a thing.

Progress- it is the process of gradually improving or getting nearer to achieving

or completing something.

Senior High School -Secondary school consists of Grade 11-12 and Core

Curriculum.

Softcopy- a legible version of a piece of data that is not printed on a physical

medium, especially as stored or displayed on a computer.

Teaching and Learning- the student prioritized not only by the teachers

themselves for they have the potential to make the teaching-learning process.

Theoretically- according to an ideal or assumed set of facts or principles: in

theory. User-Friendly- easy to learn, use, understand, or deal with. user-friendly

software.

1.7.2 Technical Terms

Agile Model – A process model that serves as a guide to programmers.

Database – A container where digital data and information is stored, organized

and secured.

Digital - Characterized by electronic and especially computerized technology

Hardware – A physical part of a computer or any device

Monitoring System -- A monitoring system is software that helps system

administrators monitor their infrastructure.

My SQL- an Oracle-backed open-source relational database management

system (RDBMS) based on Structured Query Language (SQL).

Programming – A process of constructing a system and function with the use of

a programming language and console.

Software – Instructions and data are necessary for a device to function. Sublime

Text 3 -- a shareware cross-platform source code editor. It natively supports

many programming languages and markup languages.

Web-based- System provide access to software through a computer and

internet connection.

XAMPP -- abbreviation for cross-platform, Apache, MySQL, PHP and Perl, and it

allows you to build WordPress site offline, on a local web server on your

computer.

2.1 Related Foreign Study

2.1.1 A Cloud-Based School Communication: Parent, Teacher and Student Online

Collaboration System for Monitoring the Performance of the Student.

According to Tamayo, Mark et.al (2015), developed a collaboration system for

teachers, parents, and students that will be used to monitor the students'

performance. The system is a web-based program that uses open-source application

and language development tools like PHP, HTML, CSS, Ajax, and JQuery. For efficient

operation and cost-effective service, the entire system was hosted in a cloud server

facility. The system is accessible in any updated browser such as Google Chrome,

Firefox, Safari, Opera, and

Internet Explorer.

2.1.2 A Web-Based Parent-Teacher Collaborative System for Monitoring Students’

Academic Performances in Nigerian Schools

Nigerian Schools, aimed at providing a well-designed platform for real-time

communication between teachers and parents with respect to children’s daily

performance in school. The system creates a community-like environment where

teachers and parents can interact based on the activities of the students in school.

2.1.3 Web-Based Boarding School Monitoring System.

On the other hand, Ya'acob et al. (2018), designed a web-based boarding

2.2.1 Perceived Effects of Online Learning on the Academic Performance of Senior High

School Students

According to Ramos, Shaine et. al (2021), this study aims to determine the

perceived effects of online learning on the academic performance of senior high

school students at Notre Dame of Midsayap College in the school year 2021-2022. A

quota sampling with an equal allocation was used in choosing the respondents.

Results showed that male and female respondents are equally represented, mostly 18

years old belonging to grades 11 and 12.

2.2.5 Learning at home: Parents’ lived experiences on distance learning during COVID-

19 pandemic in the Philippines

COVID-19 pandemic has closed-down educational institutions and shifted

instruction to distance learning. Modular learning addresses this learning inequality by

providing more inclusive access to education. Parents have encountered various

challenges from the new mode of learning in virtual setting. Learning Management

Systems (LMS) reinforce the learning process through online classroom environments.

2.3 Relate Foreign Literature

On the other hand, Learning Management Systems (LMS) have been the main

vehicle for delivering and managing e-learning courses. Increasing complexity of these

platforms makes LMS evaluation a hard task. Authors try to build a conceptual

framework and evaluation model of LMS through the lens of User Experience research

and practice. They conducted an online survey with 446 learning professionals and

developed a new UX-oriented evaluation model.

Software development models mainly help firms to deploy projects in a smooth

way by using specific methods and techniques. Traditional models face shortcomings of

being rigid, difficulty in accommodating changing requirements and heavy

documentation. Agile models represent a major innovation from traditional, plan-based

approaches to software engineering. The choice of a software development model

depends on project timeframe, business requirements, availability of resources, team,

client involvement and perception. S. Thomas et al (2022).

Using Agile methodologies in a financial software development environment

increases the efficiency and transparency of the development process, as well as the

stakeholders' satisfaction. Project managers must know how to adapt different Agile

frameworks to the given context of their organization. The main objectives of this study

are to investigate the main differences between project management methodologies

and agile methodologies.

Requirements: the primary stage where the requirements and project activity are

planned. The researcher’s gathered information before doing this study and informal

interview to the client to visualize the details that the system will contain.

Design: however, at this stage, the client described the layout of the system for the

Teacher, Parent, and Student. The researcher records the description from the client

and asks for some details about the system layout. From the client's description of

what could be the inside of the system. Researchers are creating the blueprint of the

system for the design that needs to be executed in the development stage.

Development: At this stage, focus is on the development of the system and also the

actual coding of the system. The system's functionality is assessed. The three major

users of the system are segregated into different functions based on their needs, from

student accounts to parent accounts to teacher accounts.

Testing: at this stage, they will test the system capability and how the system works.

They will test the appearance of this system and the things that need to improve or

change.

Deployment: where the system finish and ready to deployment. Because the

deployment stage will not begin until the system has been approved by a potential

client, the researchers completed the final phase of system construction.

Review: The last stage was the review, which was the feedback that was given by the

users to the programmer. This stage is to test the satisfaction of the users and then to

get their suggestions on what other features they need to add.

The researchers used Sublime Text 3, and XAMPP (phpMyAdmin, localhost) for

designing and development of the educational monitoring system. A Sublime Text 3 is a

program editor that allow the researchers to create system. Moreover, XAMPP a

crossplatform web server solution stack bundle that is free and open-source.

video watch. Third is the parent, who's only job is to monitor the progress of the

academics of their children. At last is the student, who can access what the teacher has

uploaded on the system. Students are allowed to download the soft copy of the

lesson/activity. In addition, the student is free to insert the finished activity for quick

recording or monitoring by the teacher and parents.

3.8 Gantt Chart for Implementation Plan

A Gantt chart shows the implementation plan of the system (See Appendix B).

In the first three weeks, communication plans, interviews, research, and

brainstorming were conducted. The planning plan was then conducted on the following

second to fifth week, including observation, project charter, feasibility study, and final

project plan. The modeling plan was then conducted in the next few weeks, where the

data flow diagram and requirements were determined, while the remaining weeks were

spent on the construction and deployment plan, during which the system was fixed and

tested and the study was completed.

3.10 Evaluation Procedure

The researchers conducted survey to determine and evaluate the system

Software functionality, content, aesthetics, and security functions. With the use of the

survey method, every function and content of the Digital Student Assessment and

Progress Monitoring in Senior High School for Santo Tomas, Batangas is analyzed to

determine whether it is still in need of improvement or properly functioning as intended

by the researchers. In the Table 4.1, the researchers utilized the 5-point Likert Scale

System to evaluate the digital application software.

RESULTS AND DISCUSSION

This chapter presents the detailed content description and structure of the

software and as well as the capabilities of the system, the evaluation of the system

software is also presented with the use of the post-survey evaluation result to

determine the functionality, content, aesthetic, and security of the system software.

4.1 Software Description and Structure

The monitoring system of the researchers, named Digital Student Assessment

and Progress Monitoring in Senior High School for Santo Tomas, Batangas, was created

for the student to give their activity by downloading and passing through the use of the

system. Moreover, in this process, the teacher will not be confused about the platform

they used to transmit the lesson or activity, nor will they be confused about the platform

they used to collect the student activity. However, the parent of the student will

monitor their child transparently with the help of this system. The system's objectives

include giving the student a quality education. There are several issues that students

face as a result of this epidemic, and these issues have an impact on their education. In

Addition, Teachers' work will speed up, and they will have more time to themselves so

they don't look stressed.

student about the lesson, video, and activity.

if it has already been watched or if the student liked the video.

4.2 Software Capabilities

The digital application software is designed to help students monitor their

academic progress. With the use of technology, the student can upload and download

the assignment or lesson, the teacher can insert the lesson and record the student

activities of each student, and those parents who are busy at work can also monitor

their child's academic progress. With the help of digital application software, the

monitoring system will be much more helpful and give satisfaction to all users.

progress monitoring system. According to the data gathered and presented above, the

respondents think that the overall system functionality got the highest mean of 4.66.

This is equivalent to 93 percent and can be categorized as excellent. Moreover, the

This led to an aggregate assessment of 4.47 percent, which is equivalent to

89.36 percent with excellent ratings.

In Table 4.4, the purpose is to assess the content of a digital student

assessment and progress monitoring system. The 10th indicator has the highest mean

with 4.66. This is equivalent to 93 percent getting an excellent rating. However, the

7th indicator got the lowest mean of 4.2 with a very good interpretation. The said

weighted mean can be transmuted to 84 percent in percentage.

Overall, the aggregate assessment has gathered a total mean of 4.47 and 89.52

percent. This is a very good verbal interpretation.

In Table 4.5, the aesthetic was measured. The lowest mean had an equal

response in the two indicators, 11th and 13th, with 4.5. This is said to mean having a

90 percent accuracy rate and getting a very good verbal interpretation. The 15th

indicator, general layout user interface, achieves the highest mean of 4.74. In

percentage, they had 95 percent and had excellent ratings.

With these things being said, the aggregate assessment of 4.61 was concluded.

This is the same with 92.16 percent and very good in verbal interpretation.

Table 4.6 aims is to evaluate the security of a digital student assessment and

progress monitoring system. According to the data shown above, respondents believe

that the legitimacy of the registration of a user's account and the overall safety and

security of using the system got the highest weighted mean of 4.62, corresponding to

92 percent and being classified as excellent. Furthermore, personal information

privacy got the lowest mean of 4.54, or 91 percent. The indication, on the other hand,

can be evaluated as excellent. As a consequence, the overall score was 4.58. This

equals 91.68 percent and is considered excellent.

Therefore, in the table 4.7, the over-all weighted mean of 4.53 with 90.68

percent and excellent verbal interpretation in the study entitles a digital student

assessment and progress monitoring system in terms of functionality, content,

aesthetics, and security.

The data represents the results of a post-survey evaluation of a monitoring

system. The evaluation is divided into four categories: Accessibility, User Experience,

Communication, and Safety. Each category is evaluated based on five criteria using a

scale of 1 to 5, where 1 is the lowest and 5 is the highest score.

In the Accessibility category, the system's interface's appealing and ease of use

received high ratings, with an average of 4.47 out of 5. In the User Experience

category, the content style, engaging content, and general content

comprehensiveness received high scores, with an average of 4.48 out of 5. In the

Communication category, the color theme, font style, and layout of the user interface

received high ratings, with an average of 4.61 out of 5. In the Safety category,

registered users' account legitimacy, personal information privacy, and security during

transactions received high scores, with an average of 4.58 out of 5.

Overall, the monitoring system received a high score of 90.68% (4.53 out of 5)

from the participants, which included students, teachers, and parents. The majority of

the participants were female (65%) and gave positive feedback on the system's

accessibility, user experience, communication, and safety.

The results suggest that the monitoring system is well-designed and has a high

level of user satisfaction. However, there may be room for improvement in some

areas, such as the speed of function response and the comprehensiveness of general

content.

4.3.3 Cost and Benefit Analysis

A cost and benefit analysis shows the break-even analysis of the system (see

Appendix D.)

In Year 0, the cost is ₱ 24,000. in Year 1, the profit is not quite higher than the

cost, wherein the Yearly Net has ₱ 13,034.88 and the Overall Net has ₱ 10,965.12. On

the other hand, according to breakeven analysis data presented in Appendix D, they

conclude that between Year 2 and Year 3, this is the part when profit surpasses cost.

Moreover, the profit in the year 4 and year 5 continues to gain profit as higher

between year 2 and year 3.

CHAPTER 5

SUMMARY, CONCLUSION AND RECOMMENDATION

5.1 Summary

The researchers developed a Digital Student Assessment and Progress

Monitoring System in Senior High School for Santo Tomas, Batangas to secure the

education of students and help them with their lessons and activities. To assist the

teacher in spreading their lesson or activity and for the inconvenient recording of the

student activity. Teachers will save time and have time for themselves. At last, the

parent of the student will not have a problem with their child not submitting their

schoolwork because they will have access to the system and they will monitor the

student properly. However, the researcher used conceptual framework model (See

Figure 2.1) served as the researcher's guide for the step-by-step planning and execution

of the entire research during the system development process. The researcher also

prepared a Gantt chart for efficient time management (See Appendix B). And as planned,

the research started in August 2022 and is planned to finish in before April 2023. The

study was completed by the planned completion date, and the processing went

successfully. For the construction of the system, the researcher used the Agile Model

(See Figure 3.1), which outlines the planned processing steps: requirements, design,

development, testing, deployment, and review.

The researcher providing the Cost and Benefits Analysis for the costs in utilizing

the system is determined as well all benefits that will occur in the future, then the two

aspects were compared, and the result was positive (See Appendix C) given that the

Return of Investment will be up to 78% in the third year and 87% in the fifth year.

5.2 Conclusion

The researcher concludes that the use of digital student assessment and progress

monitoring system has the potential to improve both teacher and student performance.

This system enables the teacher to meet the basic needs of students in education. The

parent will monitor the student's status transparently by accessing the system. The

system has proven effective in providing students the services they need for school: The

distribution of student lessons and activities; To monitor the student who downloaded

and completed the schoolwork; To collect or record student schoolwork.

Moreover, a digital student assessment and progress monitoring system can

solve some problems. The teacher will easily distribute the education needed by the

student. The gathering of the student output is not hard to do because, with the help of

the system, they use only one platform to have the needed output of students. Using

this system, the productivity of both teacher and student will improve. On the other

hand, the service of the system ensures that they provide the education that the student

needs, as long as they have devices to use and access to the system. The parent of the

student will do their job properly, and they will not be bothered by their child's

performance because they will monitor their child properly.

5.3 Recommendation

The following items are the researcher’s recommendations for the further

improvement and development of the research Digital Student Assessment and Progress

Monitoring in Senior High School for Santo Tomas, Batangas.

• Improvements of the system design and website for a more detailed presentation.

• Inclusion of taking exams and quizzes in the digital monitoring system.

• Improvement for a strong monitoring system for giving more specific monitoring

status of the students’ progress.

• Include a verify SMS notification for the updates and also recommend enhancing the

security of the application.

• Improvement of admin features for better system changes.
