IN THE MATTER OF STATUTORY DECLARATION OF OTEMA OKOT

STATUTORY DECLARATION

I, OKOT OTEMA, a male adult Ugandan of sound mind do solemnly take oath and state as
follows:-

1. THAT I am a resident of Nabuti in Mukono District.


2. THAT I was born in Uganda and named the name OTEMA OKOT
3. THAT I have been using the same name in all my academic and other official documents
4. THAT I participated in the National Identification Card Registration exercise and I was
issued the National Identification Card vide NIN No. CM1OOO1OOM1A1 IN which my
third name LAGONYA was added.
5. THAT I make this statutory declaration to verify that the inconsistences therein were
done in good faith, it is not impersonation and I am the owner of the above mentioned
particulars/documents.
6. THAT I make this solemn declaration consciously believing it to be true in accordance
with the Statutory Declaration Act.
