A patient is being treated to the posterior triangle lymphatic chain using 12 MeV electrons. 200 cGy is
prescribed to a depth of 4 cm. A 10x10 cone is being used with a custom electron insert that has a short
axis of 5 cm and a long axis of 9 cm on the patient.

a) What are the MUs that should be set for treatment (100 cm SSD)?
b) At treatment, it was discovered that the shoulders got in the way of the electron cone. The unit
therapists therefore decided to move the cone back and treat at 105 cm SSD. What MUs should
be set for treatment now?

RDF = SQRT ( RDF(5)xRDF(9) ) = ...you can use this RDF to back-extrapolate the Eq^2 f/s

Depth = 4 measn we have a PDD of 89%

Doserate = 1cGy/MU x RDF(calc’d) x PDD(89%)

MU = 200cGy/doserate = 230 MU

b...from the EQ^2 we calc’d, you can now get the SSDeff for this energy

Doserate is now = 1cGy/MU x RDF(calc’d) x PDD(89%) x ((SSDeff + 3cm)/(SSDeff + 5 + 3)^2

MU = 200cGy/doserate = 257 MU

Question 2. We are treating a boost to the seroma cavity for early stage breast cancer radiotherapy.
The prescription is 600cGy/3# using electrons and 400cGy/2# using photons. The field size for
treatment is 4 x 6 cm. For the electron field the arm got in the way and we had to treat at extended
SSD (105 cm), and for the photon boost we used an SSD of 98.5 cm. The energy is 12 MeV for the
electron boost and 6 MV for the photon boost and the dose is calculated to a depth of 4 cm in both
cases. Calculate electron and photon MUs

RDF for insert = sqrt(RDF(4)xRDF(6)) for a 6 cm applicator

From table rdf 4 under 6cm cone/applicator = 0.885

Rdf6 = 0.984

= sqrt (0.885x0.984)

= 0.933

Back interpolate into RDF chart to get a field size of 4.8 cm as

0.933-0.885/0.944-0.885 (5-4) +4 = 4.813cm

4.8-4/5-4 (82-78) +78 = = 0.8 (4)+78 = 3.2 +78 = 81.2cm

Therefore SSD eff = 81.2 cm

Doserate (elect) = 1 cGy/MU x RDF (insert) x (81.2+3/81.2+3+5)^2 x PDD (d=4)

= 1 x 0.933 x 0.891 x 0.89

= 0.740

MU = 200cGy/# / doserate(elect) = 270 MU

Photons:

Eq^2(4x6) = 4.8

Doserate = 1 cGy/MU x Sc(4.8) x Sp(4.8) x PDD (4.8, d=4)

Divide 200 cGy / doserate to get MU
