Task 2: Determining management failures contributing to the accident

Question 2
1.In the scenario, the Health Club Manager(HCM) is not aware of his health and safety
responsibility, he didn’t supervise the fitness instructor, thus fitness members followed
Unsafe practices, which resulted in the accident.
2.In this scenario, no proper assessment is carried out for seven years which is the main
failure of management contributing to the accident.
3.There is no proper train for fitness instructor about carrying out work
professionaly,which resulted in the accident.
4.Management is only concerned with satisfying their customer and don’t keep them
waiting, no attention is provided to health and safety of fitness members.
5.No proper health and safety documentation were present, they are maintaining 10 year
old health and safety policy, which was a big failure in Management.
6.No personnel protective equipment and first aid facilities were provided to the fitness
member.
7.There is no maintenance records for exercise machine, which resulted in the accident.
8.In the gym, the records of the past accidents and near misses are not kept, which
contributed the accident.
9.No proper fire drill record maintain from the management, which resulted in accident.
10.Management provide only one Fitness instructor for entire gym and he is feeling
uncomfortable, he is only one accepting overtime work. Which resulted in the accident.

1.Equipment Maintenance: The gym instructor should regularly inspect and maintain gym
equipment to ensure it is safe or not, check for locking mechanisms, loose bolts, weights
