Note: you may also include in this paragraph the status of the victim (i.e. rushed to the
hospital, was proclaimed DOA, etc.) and the status of the suspect, (i.e. immediately surrendered,
escaped, disappeared, etc) as well as the actions taken by the police officers (i.e. brought the
cadaver to PNPCL for autopsy, disposed the evidences properly, filed/prepared appropriate
charges against the suspect, etc)

1. Reference: Telephone call from a concerned citizen

2. What: A found cadaver

3. Where: Purok Katiwasayan, Bgy. Luzviminda, Puerto Princesa City,


approximately 30 km from this station

4. When: August 10, 2009 at about 8:45 am

5. Who: The victim was Romeo Cortez, 42 years old, single and a resident of the said
barangay while the suspects were Domingo Tacleso y Gascon, 50 years old, married, and his
son, Reymart Tacleso y Diaz, 26 years old, single, both are residents of the same place. The
witness was Alfred de Guzman, 17 years old, single, and the companion of the victim.

6. How: At on or about 8:45 am, August 10, 2009, this office received a telephone call
from a concerned citizen about the found cadaver at the aforementioned place. Investigation
conducted revealed that the found cadaver was identified as Romeo Cortez, 42 years old,
single and a resident of the said barangay. According to the witness, Alfred de Guzman, at on
or about 7:00 in the evening of August 9, 2009, he and the victim were in a house where they
are residing. They were talking as they are about to sleep when suddenly, DOMINGO
TACLESO Y GASCON aka DOMING, armed with bolo, and without any reason, attacked
the victim. The victim managed to evade the attack and ran for his life in fear but Domingo
chased him until reaching a small river. The victim jumped at the river to evade the attack but
REYMART TACLESO, the son of DOMINGO TACLESO struck him with a piece of wood.
Thereat, the father and son repeatedly struck and stabbed the victim. After the incident, the
duo left the area. Elements of this office are continuously conducting follow-up operation for
the possible arrest of the suspects as the case is being readied against them.

station)

3. When: July 17, 2009 at 10:30 am

4. Who: Allan Franco y Marcos, aka Boy Kalbo, 37 years old, married, farmer, and a
resident of Sitio Tarabanan, Concepcion, Puerto Princesa City.

5. Why: He was arrested by virtue of Warrant of Arrest for the crime of Rape (2
counts), docketed under CC No. 22285-86 issued on May 10, 2007 by Hon. Judge Jocelyn
Sundiang Dilig of RTC Branch 50, No Bail recommended.

6. How: Elements of Detective Unit of this office led by SPO2 William De Mesa
effected the above cited warrant of arrest and successfully arrested the subject wanted person.
Subject wanted person is still confined at Ospital ng Palawan.
