AFFIDAVIT

I, [Parent’s Full Name], aged [Age], residing at [Complete Address], do hereby


solemnly affirm and declare as follows:

1. That I am the natural guardian of the minor child, [Minor’s Old Name], born
on [Date of Birth] at [Place of Birth].

2. That _____________(old name) is my son/daughter and mother of the minor


child namely Ms._____________(mother’s name).

3. That name of my son/daughter is as ____________(old name) according to


the birth certificate issued by the Government.

4. That for reasons of [State Reason], it is necessary to change the name of the
minor from [Minor’s Old Name] to [Minor’s New Name].

5. That I am changing son/daughter’s name to _______________(new name).

6. That henceforth, for all legal and official purposes, the minor shall be known
by the name [Minor’s New Name].

7. That this affidavit is made for the purpose of registering the change of name
of the minor child and for all other lawful purposes.

8. That the contents of this affidavit are true to the best of my knowledge.
