All schools are advised to conduct the initial screening of documents as to eligibility requirement of interested Master ‘Teacher I and II applicants. Requirements a. Letter of intent addressed to the SDS; b. Duly accomplished and notarized PDS (CS Form No. 212, Revised 2017); c. Photocopy of valid and updated PRC License /ID; d. Photocopy of scholastic/academic record such as but not limited to Transcript of Records (TOR), including completion of graduate and post-graduate units/ degrees; e. Updated Service Record /COE; f. Photocopy of the duly accomplished Performance Ratings in for the last three (3) years g. Notarized SF7 h. In accordance with RA 10173, otherwise known as the “Data Privacy Act of 2012, an application code shall be assigned to each applicant to protect the applicant’s personal information. 8. No applicant shall have the same application code. 10.Only applicants with full teaching loads are qualified to apply for promotion/reclassification to MT positions. 11. Applicants are advised to bring original copies of their documents during the evaluation for verification purposes. No additional documents shall be accepted after the said deadline. 12. The applicant assumes full responsibility and accountability for the completeness, authenticity, and veracity of the documents submitted. The Division Sub-committee shall check and verify the completeness, authenticity, and veracity of the documents submitted. Any false and fraudulent documents submitted shall be grounds for disqualification. 13.Upon receipt of the applications, an initial assessment of the applicant's qualification vis-A-vis the CSC approved QS shall be conducted by the School HRMPSB. They shall assess whether the applicants have submitted the complete documents and meet the minimum qualifications for the position. 13. The School HRMPSB shall notify all applicants of the results of the initial assessment through a written notice serve through official communication channels, such as but not limited to personal service, postal mail, courier service or electronic mail using official email accounts, official social media accounts and other means of communication that can be verified, recorded, and preserved. Telephone calls and short messaging services may be used only to accompany the aforementioned official communication channels. 14.The Chairman of the School HRMPSB shall assume full responsibility and accountability in the result of the initial evaluation, and therefore, shall be responsible in responding to queries and clarifications pertaining to the initial evaluation. 15.The following shall be submitted to the Records Section, DepEd Antipolo City, C. Lawis Extn., Brgy. San Isidro, Antipolo City on or before April 24, 2024 from 8:00am to 5:00pm: Result of the assessment duly signed by the School HRMPSB Minutes of the proceeding Soft and Hard Copy of documents of all applicants Notarized SE7 List of Teachers per subject area (JHS) pao ee 16. An Open Ranking System shall be adopted to ensure transparency in the process and results of the comparative assessment wherein applicants are present to witness the actual evaluation for which the scores of the applicants present thereat shall be disclosed to all attendees. Each applicant shall also be given an opportunity to ask questions and seek clarifications on the results of their individual assessments and acknowledge their individual results during the Open Ranking. 17.The assessment process shall be documented using the Minutes of Deliberation. Queries and clarifications pertaining to the individual results as well as the response and action taken shall be reflected in the documentation. 18. After the judicious and objective assessment of the qualified candidates, the Division HRMPSB shall convene and deliberate on the results of the assessment and submit to the appointing authority the duly signed CAR within the prescribed period. 19. It is understood that qualified MT in Elementary Level for promotion are willing to be transferred to any school within the district where the vacancy exist 20. Qualified applicants who will not be promoted may be recommended for reclassification, provided that the following parameters for reclassification is followed: a, Elementary ~ total Master Teachers position shall not exceed 10% of the authorized teaching position in the school and district, to wit: © Master Teacher I - shall not exceed 6.6% of the total number of authorized teaching position in the school © Master Teacher Il - shall not exceed 3.4% of the total number of authorized teaching position in the school b. JHS - the ratio of allocation of MT per department is 5:1 (there must be a total of 5 teachers to be supervised for every MT) 21.
