(1) Please indicate your Time-in and out for the said date period; I hereby certify that the entries on this time record was made personally by the undersigned and that the same are true and correct.
(2) Indicate type of Leave on the Remarks Section;
(3) Overtime will start after the 8 Regular Working Hours;
(4) In case of Leave/Absence, attach the required Absence/Leave Request Form; Employee's Signature Department Head/Project-in-charge Division Head
(5) In case of Overtime due to continuous pouring, indicate the volume poured; and
(6) For other causes, attach the Accomplishment Report/Trip Records.
