Below you will find a series of academic and behavioral parameters to take into account for your current school period
attitudinal evaluation. Write in front of each parameter the evaluation from 1.0 to 5.0 (using decimals). At the end add up
all the data and divide by 12 to get your rating; the questions at the end must be solved.

No PARAMETERS MARK
1 I actively participated in the development of the class by contributing appropriate ideas, on his or her own
initiative
2 I have fully complied with all the tasks, workshops, works and/or activities set for the development of the
class in a personal and responsible manner.
3 I have brought on time the material and/or activities proposed for the development of the classes (notebooks,
guide, book, workshops, printed sheets or images, improvement plan, among others).
4 I have attended all the classes and, if not, I have presented to the teacher the excuse given by the teacher
for discipline coordination within the three days established in the Behavioral Manual.
5 I have presented and passed the evaluations on the dates established by the academic coordination and the
teacher in an honest way without any help.
6 I have entered classes on time according to the schedule established by the institution.
7 I have presented and developed the improvement and/or reinforcement plan (work and presentation on the
assigned date).
8 I have maintained a good personal appearance in class by wearing the complete uniform without omitting
and/or changing clothes, according to the class schedule.
9 I have complied with the rules of the Behavior Manual such as maintaining a proper haircut, hairstyle, no
dyeing, locks or related, do not wear clothes that are not part of the uniform, do not chew gum, do not eat in
class, etc..
10 I have respected the opinion, performance and work done by my classmates.
11 I have respected the teacher and the development of the class as such without being called to attention.
12 I have used technological tools for the development of the classes only with the teacher's authorization (cell
phone, headphones, virtual notebook, etc.).
