This Plan is Describe the proper procedures and preparations to protect the health and safety of all Dr.
Concrete personnel who shall work in confined spaces. All guidelines contained shall be regarded as the
minimum requirements for Dr. Concrete owned or managed sites.
Ventilation is to be used to introduce fresh air to a confined space or to exhaust contaminants created by
work performed such as welding, abrasive work, oxygen / acetylene cutting, coring in materials
containing silica, etc.

2. SCOPE OF APPLICATION
This Plan serves to outline the safe operating specifications as well as procedure and guidance required
for project related to Occupational Safety and Health Requirements for confined space.

3. PROCEDURE FOR ENTRY INTO CONFINED SPACES.


3.1. Confined Space Attendant

The Confined Space Attendant shall perform the duties listed in Section 3.0 paragraph 3.6. Additionally,
where the space contains residual fluids (e.g. water) into which a person might fall so that his face and
BA are submerged, he shall be assisted by a rescue person who shall be stationed at the opening in
addition to the Confined Space Attendant, equipped to make an entry wearing BA so as to give
immediate help, if he considers it safe to do so.

3.2. Entrants

Entrants shall perform the duties listed in Section 3.0 paragraph 3.5. In addition, they shall wear
approved positive pressure BA and be trained in its use.

 Canister respirators shall not be used in any circumstances. Air-supplied apparatus shall be used.
 All personnel expected to wear BA and carry out work at the same time are recommended to be
clean-shaven in order to provide a good seal between mask and face.
 If it is not possible to class the space as ‘fit for entry without BA’, entry WITH BA may be
permitted, subject to the criteria in Table 1 and in accordance with the additional precautions below:
- Approved positive pressure BA is worn.
- Authorisation for ENTRY WITH BA has been given.
- Where practicable, retrieval gear is worn.
- The Entry Attendant must be stationed outside the space entrance at all times.
- Rescue and reviving equipment and qualified personnel shall be readily available.

3.3. Ventilation

Ventilation is to be used to introduce fresh air to a confined space or to exhaust contaminants created by
work performed such as welding, abrasive work, oxygen / acetylene cutting, coring in materials
containing silica, etc. Ventilation is to be done as follows:

 Select a fan capable of 8 to 10 fresh air exchanges in the space per hour.
 Ensure that the fan or fans are in good working condition.
 Ensure that fans are grounded and bonded to the confined space.
 Follow the instructions of all safety and warning labels on fans.
 Locate supply fan(s) so that only clean fresh air will be drawn into the confined space.
 Ensure the supply fan is located well away from building exhausts, generator exhausts, automobile
exhausts or any other toxic or flammable substances.
 Direct the ventilation duct so that good clean fresh air is provided throughout the confined space
including alcoves where contaminated air may be trapped.
 Position the inlet for exhaust ventilation next to the source of the contaminant. (Welding, cleaning
with solvents, concrete coring, etc.)
 Ensure that exhaust fan outlets are positioned so that contaminants cannot be drawn back into the
confined space by the intake ventilation.
 Ensure that intake and outlet ducting does not get kinked.
 Keep ducting as far away from work areas as possible in order to avoid damage.
 Ensure outlets are positioned where natural air currents will disperse quickly and not create a
danger to other people.
 If there is any chance that exhaust air could be flammable, ensure that there are no ignition sources
anywhere near the outlet.
 Continue to ventilate the confined space for the entire time that entrants are working in the space.
 When entrants leave a confined space, for whatever reason, the confined space atmosphere must be
tested before any employee re-enters the space.
 Test the confined space atmospheric levels prior to the start of work each day.
 If the confined space is not oxygen deficient, appropriate air purifying respirators should be used, in
addition to ventilation, to help protect entrants if the atmospheric contaminants are not Immediately
Dangerous to Life or Health.
Use of ventilation Equipment
Ventilation shall preferably be accomplished using a positive method of mechanical ventilation that is
arranged to:

 Introduce sufficient fresh air and remove contaminants from all pockets or corners of the confined
space
 Avoid re-circulating contaminated air
Even after the confined space is cleaned and ventilated, the mechanical ventilation equipment must be
kept operating to provide secondary protection:

 In case of accidental introduction of harmful substances


 To remove contamination or heat that may be produced by the work (e.g., welding & cutting,
painting, coating)
The atmosphere must continue to be monitored for hazardous atmospheres while personnel are inside
the confined space.
Ventilation Air Source
The ventilation air used will be from either:

 An approved electrical blower for a confined space area, or


 An air driven blower
The air intakes for these devices shall be located where no contaminants may enter the stream.
Disposal of Confined Space Atmosphere
Outlets for power driven blowers, pneumatic air inductors, or air/steam inductors used to draw vapours
out of a confined space must be directed to a safe place far from possible sources of ignition.
General Ventilation
When welding (especially MIG/TIG) or other work is to be carried out in a confined space, precautions
shall be taken to ensure that an adequate flow of fresh air is available and that welding fumes are
removed. Great care shall be taken to ensure that the airflow is always away from the breathing zone of
the welder and that contaminated air does not 'short circuit' back to the welder.
Where exhaust ventilation is used in a confined space, the extracted air shall be diverted well away from
the confined space so that replacement air shall be fresh. When adequate ventilation cannot be
provided, approved breathing apparatus or a suitable facemask supplied with fresh air shall be worn.
Compressed oxygen or instrument air shall never be used to ventilate a confined space.

3.4. Lighting and Portable Tools

Temporary Lighting
The following precautions shall be observed when using temporary lighting:

 Adequate and suitable lighting shall be provided for entry and work in a confined space. Access and
passage into a confined space shall be provided with illumination of not less than 50 lux. All

portable
hand-held lightings provided in confined spaces shall be operated at a voltage not exceeding
alternative current (AC) 55 volts between the conductor and earth or direct current (DC) 110 volts.
 Temporary lights shall be equipped with guards to prevent accidental contact with the bulb.
 Temporary lights shall be equipped with heavy-duty electric cords with connections and insulation
maintained in safe condition.
 Temporary lights may not be suspended by their electric cords unless cords and lights are designed
for this means of suspension. Splices shall have insulation equal to that of a cable.
 Temporary lights and electrical services should be protected by an earth leakage circuit breaker
(ELCB).
Working spaces, walkways, and similar locations shall be kept clear of cords so as not to create a
hazard to workers. All electrical equipment should be protected by an ELCB.
 All cables must be hang with vertical hanger above ground and the Generator never taken inside the
Confined Space.
 Particular attention must be paid to the protection of cables passing through doors and entry points.
Tools
If the atmosphere inside the confined space requires BA to be worn (see Table 1) because of the
presence of hydrocarbon, no electrical tools must be used by the entrants. Air-driven tools only may be
used. Tools with the potential to produce sparks, e.g., grinders or needle guns, shall not be used.

2. Before any welding, cutting, and grinding may be carried out in a confined space; the space shall be
proved completely gas free, and free of all flammable residuals.

3. A schematic diagram to show the sequence of planning for Confined Space Entry is shown in
Appendix A.
4. JOB COMPLETION
If the vessel/tank is unmanned for any period, the entry points shall be barricaded off. The Entry Permit
must be cancelled upon completion of the entry and after all entrants have exited. At the end of a job, a
thorough check must be made by Area and Performing Authority to ensure that no personnel, tools or
equipment have been left behind.
