IDENTIFYING INFORMATION:
Miss Xyz is a 20 years old female. September 2000 was her birth. She is a first-year B’s
Psychology student at the University of Karachi. Her father, who is 55 years old, recently retired
From a pharmaceutical company. He completed his master’s degree from Karachi University.
Her mother, who is 45 years old and a housewife at the moment, graduated from Karachi
university’s pharmacy program. The client is from a middle socioeconomic status. The client is
middle child of the three female siblings. Her nuclear family lives in Gulshan-e-Iqbal, close to a
disco bakery. She has been living in this location for the previous four years. She previously
The client is volunteer for the clinical assessment for a final year project purpose. She has a
INTERVIEW INFORMATION
The client suffers from a physical condition called khyposis, which is a spinal disorder in which
excessive curvature of the spine results in an abnormal rounding of the upper back and also
fingers are not straight. She used to get therapy for her medical condition but not anymore. Her
sister is also suffering from the same disease. She said it might be due to cousin marriage as his
mother and father were cousins. Her relationship with her sister is normal but she does not tell
most of her things to her sister but shares a little with her elder sister. Her elder sister is now
married. Her relationship with his parents is normal. She said that his school life was not good
and she changed several schools only because she lives in a rented house and whenever she
changes house, she also changes her school. She gave me the names of at least four schools that
had been changed. She didn’t have many friends at school, she used to talk to all her classmates
but she only had one friend at school. She had no friends of the opposite gender, she didn’t even
talk to her male cousin. She also revealed that he has stage phobia. She said that her anxiety
during presentations is at an extreme level and she feels anxious if someone stares at her for
more than 3 seconds, accordingly she cannot handle the attention of others. Even if someone
pays attention to her, she feels awkward. Her father is diabetic and her uncle is schizophrenic.
The rest of the family does not have any mental disorders. She was very cooperative, tried to
answer every question, but was a bit nervous maybe just because of the first interaction or as she
said she can’t handle attention from others and she feels anxious if someone stares at her for
TESTS ADMINISTERED
Rorschach (ROR).
The client was confident and relaxed during the assessments due to the repo
opposite gender. In some tests the client was little bit anxious especially in human
PSYCHOLOGICAL EVALUATION.
The client score in intelligence scale indicates the IQ of 97 which is average IQ level according
to her 20 years of age. Her verbal IQ is 133 and performance IQ is 77 which indicates that
According to Rorschach, the client has superior intellectual ability and excessively high
aspiration level since she is unable to devote enough of her own creative energy to her efforts in
order to produce a stable outcome. Additionally the client’s ability to see the environment in an
impersonal. The client is not able to use her inner resources to constructively solve day-to-day
problems since the tension is too great and the client struggles with emotional regulation. The
client’s feels anxiousness and discomfort when someone is paying attention to her. The client is
not managing her mental energy or drive in a way that would be expected of someone in a
previous chronological age. ROR also symbolizes forces most likely to engender anxiety if
permitted direct expression therefore she represet the most repressed forces in the personality
structure. ROR also shows disturbance when the environment stimulates touch in the area of
affectionate need which She disclosed in her case history that she becomes easily distracted by
her surroundings and becomes uneasy when someone is staring at her for longer than three
seconds. Furthermore Rorschach shows that the client is genuinely concerned about her physique
and raises suspicions about the richness of her inner life and acceptance of who she is.
The client's HFD reports show's anxiety, emotional dependency and discomfort which is also
supported by case history and other tests. HFD is also indicating sexual conflict which is
supported by case history that according to the client she doesn’t have any friends of the opposite
Gender and She doesn’t even speak to her male relatives. HFD results also indicating that Client
is facing difficulty in social contact and unable to perform infront of others which also
supporting from case history that the client is unable to bear the attention from someone for more
than 3 seconds . Also the results shows aggressiveness which is also shown in the results of
ROR.
TAT indicates the client’s desire to develop a strong and close relationship with others. The
environment was Perceived as mostly rejecting, uncongenial, and aggressive. The client’s
relationship with others are unsupportive, restrictive and rejecting. TAT also indicates client’s
The ISB indicates that the client is strongly attached to her parents but she is unable to do
Something for them and she has considerable conflicts over her inability to perform her
responsibilities. ISB also shows that She is unsatisfied from environment and unhappy from way
the people perceive and not giving space to each other and express more negative attitude
towards them. The client seems introvert and her traits show social anxiety and she had a fear to
speak in social situations and also the client is struggling with antisocial tendencies . ISB also
gives an indication that the client is seriously maladjusted in family, social & sexual area.
TENTATIVE DIAGNOSIS.
F40.10 Social Anxiety Disorder. The client is facing difficulty in social contact and unable to
perform infront of others. She is unsatisfied from environment and unhappy from way the people
perceive and not giving space to each other and express more negative attitude towards them.
The client’s feels anxiousness and discomfort when someone is paying attention to her especially
for more than 3 seconds that’s she avoid to interact with other’s. These all are the symptoms that
PROGNOSIS.
RECOMMENDATIONS.
she need a counselling and , I advise her whenever client feels anxious close her eyes and take
long deep breaths and make an anchoring point in her thumb and whenever she feels uneasy or
anxious press the thumb and recall any positive event before it.
SUMMARY.
On abc subject, ROR, TAT,HFD, and ISB test were administered and recommendations were
given.
