Instructions to Candidate Please use a pen not pencil to complete this questionnaire, write your
answers in the space provided next to each question
Read the questions carefully. Your answers must be in your own words and in
sentence form unless the question asks you to list or name
If you have a concern in completing this assessment speak to your assessor
privately
If you find it hard to learn, you can ask for some changes to the way the
assessment is done. These changes are called reasonable adjustments, and
they help you to show what you know and can do in the assessments.
Your assessor will mark your work Satisfactory (S) or Not Yet Satisfactory
(NYS). All answers must be answered to a satisfactory level before continuing
to the observation assessment
Your assessor will give you feedback on your result. If your work is NYS you
will be given time to reattempt any incorrect answers. You have up to two
attempts in total
If your view that the results of your assessment are not accurate or
unreasonable, you can ask for a re-assessment or an appeal. A re-assessment
checks your paper again for marking errors. An appeal challenges assessor’s
decision with your evidence.

without diminishing the unit of competency if required

the candidate and provide a second opportunity
If the candidate is unsatisfied with the result, advise the candidate that they
have a right to appeal the decision by accessing the RTO appeals policy
All documentation forwarded to the RTO admin team within three days of
completion
Any issues experienced with the assessment documentation is to be reported
to the RTO Compliance Manager
