Greetings!

I am writing your good office relative to the concern of our client Unistar Package
Printing Corporation (Unistar) represented by its President Ma. Isabel C. Pontejos with
office address at Unit 107 VFP Building 2, Veterans Center, Western Bicutan, Taguig City,
relative to their concern for their failure to pay the monthly contribution of their
employees thereat beginning April 2004 to May 2010 our clients already aware of the
salient features of the PhilHealth and thus are very willing to comply with the
requirement needed by the company in order that they be considered a good employer
and their employees to be paid their supposed contribution accordingly in order to
enjoy the benefits of a member of good standing with the PhilHealth.

Aware that the payments of contributions is mandatory pursuant to R.A. 7875 as


amended by R.A. Nos. 9241 and 10606.

With all due respect and the indulgence of your office our client seeks your
understanding and humane concern relative to the payment of their unpaid
contributions and their question that we are trying to rely on you are the following:

1. Can Unistar unpaid premiums be paid on installments so as not to hinder its


corporate operations considering the fact that their financial sources is limited;
2. Can your office consider their resigned employees or have resign abandoned and
or abandoned their work such that upon their resignation or termination, their
payment should be stop or cancel so as to deplete the corporation funds for its
day-to-day operation;
3. Can the penalties and other charges in the non-payment of their monthly
premium be waived or condoned by PhilHealth so as not to burdened them with
the huge amount representing such charges and penalties?
4. Can they submit to your office the resigned, terminated or when an Absence
without Leave employees/workers effective reconsidered resign, terminated or
absence without leave;
5. Can they request your office to have the installment of unpaid premiums
scheduled on 24 installments but the outstanding monthly premiums shall be
paid monthly shall be paid starting this June 2024

If it would not be too much on asking a favor from your office, would it be possible
that the scheduled installment fees be differed in the meantime or shall we say your
office will give a breathing space for them to pay their arrear unimpaired

Sir, I beg for your kindest understanding and utmost consideration of the plight of
our client Unistar by allowing them the best terms your office can afford because it is to
the best interest of them and their office to have a win-win solution.
