A The contractor shall provide and maintain all the necessary protective and safety helmets,
rubber boots, safety shoes and standard colour T-shirt for all the site staffs and workers to be
worn at all time throughout the construction period.
B Allow cost for complying with all the Health and Safety precaution for the works, including
provision for all the necessary equipment, material and labour in compliance with all the health,
safety and security requirements.
C To provide full time, qualified and experienced Site Safety Supervisor as required by the
Department of Safety and Health (DOSH) to the approval of the C.A.
D Allow cost for the provision of the Safety Vest (as per AIR SELANGOR's specification and
requirement) for each of the site staff and to be worn at all the time throughout the construction
period.
F To provide the traffic management control system to the requirements of the relevant
authorities.
G Allow cost for liaison with the Authorities for the permit application, site registration and working
within the road / existing utilities reserve to meet the requirement of the Safety and Health and
Road / Utilities Authorities (DOSH, DBKL, MPAJ, TNB, TM, etc.) All related costs including the
application fees and mandatory charges shall be borne by the Contractor. Bank Guarantee or
any other form of deposit shall be borne by AIR SELANGOR.
H Allow cost for the provision and maintenance of all the traffic or warning signs, barricades
NYLEX road barriers, signal and blinking lamps and personnel to guide movements of traffic
day and night during the duration of the works. (All the road traffic signs are to be of the AIR
SELANGOR's standard drawings.) Minimum requirements for each works area:

J To attend and complete the full day of training on Air Selangor NIOSH Safety Passport Course
for all the employees or workers of the contractor and sub-contractors who are required to
work at site(s).
K To obtain the Air Selangor Niosh Safety Passport Card after completion of the training. course.
L To ensure that all the employees and workers who are engaged in the work at the site (s) have
a valid Air Selangor Niosh Safety Passport Contractor's card issued by Air Selangor. The card
shall be displayed at all times when working at the site (s) and/or at any Air Selangor premise
(s).
M Report analysis, report writing and presentation To provide a copy of the Air Selangor NIOSH
Safety passport to the Pengurusan Air Selangor Sdn Bhd management team.
N To provide and ensure all the workers involved in the confined space activities possess the
valid competencies as per required by the Industry Code of Practice For Safe Working In
Confined Space 2010 as per below:
P All competent persons shall possess the valid competency card issued by the National Institute
of Occupational Safety & Health (NIOSH) or any DOSH-recognized training provider.
Authorized Gas Tester and Entry Supervisor must possess the competency certificate issued
by the Department of Occupational Safety & Health (DOSH).
Q To provide the personal protective equipment (PPE), confined space equipment and
emergency equipment as per require but not limited by the Industry Code of Practice For Safe
Working In Confined Space 2010 and any related regulations as per below:
(blank) a) Basic PPE including safety helmet, safety shoes, and safety vest.
(blank) b) Special PPE including safety harness with double lanyard and etc.
(blank) d) Atmosphere testing and monitoring equipment (5 gases of Cl2, O2, CO, H2S and CH4).
(blank) f) Equipment for safe ingress and egress such as ladder, winch and tripod.

R To provide and post at site the adequate danger sign to indicate the work is being carried out in
the confined space, entering the confined space is prohibited unless authorized by the Entry
Supervisor.
S To prepare and submit the working in confined space documentation. The preparer shall at
least attend the confined space awareness training. All the documentation submitted shall be
endorsed by the contractor's Authorized Gas Tester:
(blank) e) Other related plan in case of using special installation or machinery i.e. scaffolding, mobile
crane and etc.
(blank) Based on the risk assessment, practicable hazard control shall be established and
implemented.

V The contractor shall provide the appropriate equipment as follow:-

controls do not adequately protect authorized entrants.

to be able to exit the confined space quickly in case of emergency.

W Ventilation shall be provided continuously throughout the period of the occupancy in the
confined space.
X The contractor shall ensure that his authorised entrant intending to work in confined space are
certified physically and mentally fit determined by an occupational health doctor (OHD shall
issue a health fitness certificate that shall be valid for a period of two (2) years.
Y To provide an adequate fall protection device including safety harness to all the related worker
during working above 3 meter. Fall protection device shall be approved by the Department of
Occupational Safety & Health (DOSH).
Z To provide an adequate and proper A ladder (if any). The ladder shall be approved by the
approved authority bodies i.e. SIRIM and etc.
AA To provide a suitable working platform such as scaffolding complete with the erection &
inspection tagging.
AB To provide a competent erector / inspector recognized by DOSH for the scaffolding structure
AC Erection of the scaffolding more than 15 metre must be designed and drawing by the
Professional Engineer.
AD To provide an adequate and proper chemical container complete with the labelling as per
require by the Classification, Labelling And Safety Data Sheet 2013 (CLASS 2013).
AE To provide storage area for chemical complete with warning signs that is suitable and
adequate as per stated in the Safety Data Sheet and CLASS 2013
AF To provide an adequate and proper PPE (approved with DOSH-SIRIM) as per stated in the
Safety Data Sheet.
AG To provide an adequate documentation for each chemical used including Safety Data Sheet
and Chemical Register.
AH To provide training related to chemical handling to workers as per requirement in Use and
Standards of Exposure of Chemical Hazardous to Health (USECHH 2000)
AJ To appoint assessor for conducting Chemical Health Risk Assessment (CHRA), Chemical
Exposure Monitoring (CEM) (if necessary) & Medical Surveillance (if necessary) as per
USECHH 2000
AK To carry out proper chemical waste disposal as per require by the related regulations including
the Environmental Quality (Scheduled Waste) Regulation 2005 and etc.

AR To provide shoring or suitable temporary structure for the excavation more than 1.5 metre
depth.
AS To provide suitable safe access into the excavation site such as ladder.
AT To provide the AESP and AGT if depth of the excavation works considered as confined space,
more than 1.5 metre.

AY To conduct the baseline survey and identification of the ACM material By the Competent
Asbestos Consultant: P406 Supervision and Management of the Safe Removal and Disposal
of Asbestos. Proficiency Certificate No. 180111/004 and Registered Chemist AMIC
1095/2640/96/99 and Accredited Laboratory SAMM 213.
AZ To liaise with the relevant authority (DOE & DOSH) for the Asbestos Removal (Surat Tanpa
Halangan).
BA To conduct the Chemical Health Risk Assessment and to provide CHRA report.
(blank) b) Comply with the Reg 9 of the USECHH Regulation 2000 (Use & Standards Of Exposure Of
Chemical Hazardous To Health Regulations 2000).
BB To conduct the Air Monitoring while maintenance work is in progress (test for the airborne
