Near Miss report

5. Electrical: A worker accidentally cuts into a live electrical line,
causing sparks, but no one is electrocuted or injured.
6. Scaffolding: Workers notice a wobbly or unstable scaffold
platform but manage to stabilize it before it collapses.
7. Vehicle Incidents: A construction vehicle, such as a forklift or
backhoe, nearly collides with workers or another vehicle on
site.

o Names and roles of individuals involved in or witnesses
