Question 1- Question 14: Information of York University. The above information applies to all York University
employers.

Question 15 – Question 20: Contact information of “the Employer”.

Note: In the setting of designated learning institution, the “Employer” could be:
- The person who signs the invitation letter issued to the IVRT, or
- The person who is in charge of the hiring process

21. Click on the drop-down menu, and choose the one that best describes the IVRT.
22. This box would complete itself once the Question 21 is chosen. Please refer to the link below for reference for the LMIA

23. Explain how the job can’t be completed by local worker (Canadian citizen/permanent resident), e.g. this job has to be
accomplished by this IVRT who has exceptional international experience and expertise in a particular field.
24. It would be the same title as indicated on the invitation letter issued to the IVRT.

Each NOC code represents different job titles. Please select the one that best describes the job title.

DECLARATION OF EMPLOYER
Please ensure all the boxes are checked, and then type the employer’s name where indicates “Name of
employer”. The signature can be left blank, as the form will be submitted to CIC electronically via email.
To submit: Upon completion of the application form, hit the button on the bottom of the form.
After submission, you will receive an automatic acknowledgement from CIC of receipt. Please
provide a copy of this completed form to the IVRT for his/her work permit application.
