POSTNATAL WARD: Tasks and Subtasks 2024/2025

1. Task: Patient Handover and Communication


o Subtasks:
 Conduct shift handover and receive patient updates.
 Receive and review handover reports from the morning shift.
 Receive and review handover reports from the evening shift.
 Collaborate with the multidisciplinary team during rounds.
 Communicate any changes in patient status to the physician.
 Communicate patient status to the on-call physician as needed.
 Educate family members about patient status and care plans.
 Prepare for morning shift handover by updating reports.
 Communicate effectively with the healthcare team.
2. Task: Patient Assessment and Monitoring
o Subtasks:
 Perform initial head-to-toe assessments of mother and baby.
 Conduct patient assessments at the start of the shift.
 Monitor and document vital signs of mother and baby regularly.
 Monitor for any signs of postpartum complications.
 Respond to any patient alarms or emergencies.
 Monitor for signs of neonatal distress.
 Perform routine checks on newborns (e.g., APGAR scores, jaundice
screening).
 Perform patient safety checks.
3. Task: Medication Administration and Management
o Subtasks:
 Administer medications to the mother as per prescription.
 Administer medications to the newborn as per prescription.
 Manage IV fluids for the mother if required.
 Assist in procedures such as epidural or spinal anesthesia management.
 Administer vaccinations to the newborn as scheduled.
 Assist with pain management for the mother.
4. Task: Patient Care and Hygiene
o Subtasks:
 Conduct daily hygiene care for the mother.
 Assist with newborn care (e.g., bathing, diaper changes).
 Ensure the mother is comfortable and assist with repositioning.
 Assist mothers with breastfeeding and feeding if necessary.
 Change dressings on any postpartum wounds or surgical incisions.
 Provide perineal care for the mother.
 Educate mothers on newborn care and personal hygiene.
5. Task: Diagnostic Tests and Procedures
o Subtasks:
 Draw blood for lab tests for the mother and newborn.
 Prepare mothers for any diagnostic tests or procedures.
 Perform newborn screenings and tests (e.g., hearing tests, blood spot
tests).
 Collect and send samples for lab tests.
6. Task: Patient Support and Safety
o Subtasks:
 Provide emotional support to mothers and families.
 Assist with early mobilization of the mother post-delivery.
 Ensure all tubes and drains (e.g., urinary catheters) are functioning
properly.
 Ensure all emergency equipment is ready and functional.
 Manage and restock supplies for the unit.
 Ensure patient privacy and confidentiality.
 Provide education on postpartum care and newborn care.
7. Task: Record Keeping and Updates
o Subtasks:
 Update care plans based on assessments of the mother and baby.
 Update the care plan and patient records regularly.
 Maintain accurate and thorough patient records for both mother and baby.
8. Task: Infection Control and Education
o Subtasks:
 Adhere to infection control protocols.
 Educate mothers on infection prevention for themselves and their
newborns.
 Participate in continuing education and training.
