TRANSCRIPT REQUEST FORM

You only require a transcript if:


• You are applying to a university/college/trade school and need a transcript
before the marks are uploaded to OUAC/OCAS. OR
• You are applying for a scholarship, bursary or other financial support for
post-secondary education.

You DO NOT require a transcript if:


• You are not graduating from secondary school after this credit.
• You are a Peel student, as your marks are already in the Student
Information System and a Master Grade Summary has already been
forwarded to your day school with your marks.
• You are a non-Peel student, as you have your report card which can be
taken to your day school Guidance Counsellor and a Master Grade
Summary has already been forwarded to your day school with your marks.

1. Print and complete the form below before the last day of the program, i.e. International
Languages, Night School or Summer School.
2. Provide a full and accurate mailing address for your home or the institution to which the transcript
will be forwarded. If possible add an email address if the institution will accept an electronic copy
of the transcript. OR
3. Scan your form using a scanner or a scanner app OR take a picture of the form on your Smartphone
4. Email this request to [email protected] with the Subject message of “Transcript Request”.

embossed with the Continuing Education seal. Transcripts are then placed in a sealed envelope and
embossed with the Continuing Education seal. Opening this envelope voids the transcript.
