AFFIDAVIT FOR ADDRESS PROOF


I, B. Vijayakumari D/O. Srinivas Rao, aged about 35 years, Occ: Housewife,
R/o. Plot No. 2106, 2nd block, First floor, Janapriya apartments, Satya Nagar,
Kothapet, R.R.District do hereby solemnly affirm and state on oath as follows:-

1. I am the Deponent herein and as such I am well acquainted with the facts of
this affidavit.

2. I submit that I am the absolute owner and possessor of the above House
bearing Plot No. 2108, H.No. 4-1-488/2108, 2nd block, First floor, Janapriya
apartments, Satya Nagar, Kothapet, R.R.District. That Smt. Kothuru
Mahalaxmi, W/o. Sri. Kothru Rajashekar, Aged about 36 years, Occ:
Housewife is my Tenant and she is residing in the above said house along with
her parents and children on rent.

Hence, I pray that this Hon’ble Court may be pleased to consider this affidavit
as residential proof of Smt. Kothuru Mahalaxmi, W/o. Sri. Kothru Rajashekar
for which I am giving his declaration.

The above facts are true and correct to the best of my knowledge and belief
that in case my above statement is proved to be false, I will be held responsible for
the same and action may be taken against me U/S. 199 & 200 IP./Cr.P.C.
